Stacked on #3984 → #3983 → #3982 → #3981. Child-only diff: src/activity_bus.py, src/activity_rows.py, tests. Owner's constraint, 2026-09-06: activity visibility is enforced at transport/storage level, not merely at render time — so the audience is decided where the state is, before any transport exists.

What

  • Two fields, two questions (owner, second message): audience = who may receive (owner | room | selected_members | system), projection = what they see (TASK_STATUS | RUNTIME_DETAIL | AVAILABILITY). TaskActivityState.visibility = {lifecycle: room, telemetry: owner} by default.
  • Every projected row carries audience and projection: lifecycle transitions (queued, picked up, waiting for you, replied, failed, cancelled) → room; observations (thinking, working, tool, status) → owner. A WAITING an observation caused is still lifecycle.
  • shared_projection(state) — phase, worker, generation, timing, message id: what a room may know (someone is on it, where it stands). No summary, no steps. private_projection(state) — the shared fields plus summary, seq, session: only the owner's own clients. C carries the first through a room state event and the second through an owner-scoped stream; a member never receives the private payload.
  • The row writer persists both when given; a row with no stated audience carries none (the client's existing rule applies; today's file is owner-only by construction).

Evidence

tests/activity-bus.test.py   Ran 19 tests in 0.027s OK   (16 on #3984; head b0f6545 renames scope → audience + projection)
tests/agent-activity.test.py Ran 39 tests in 0.280s OK

Checks: lifecycle rows are the room's TASK_STATUS and observations the owner's RUNTIME_DETAIL (including a WAITING caused by an observation); the shared projection carries no summary and the private one does; the writer persists the scope and a row without one carries none. review-checks: PASS.

Worst case

One optional field on rows the client ignores today; no transport is added here.

Approving at b0f65459. Child-only layer c1ca07d6..b0f65459 — 3 files, +69/-5; #3984 confirmed as ancestor. Bus suite 19 (was 16), agent-activity 39, both green.

Note on the head: review-guard read 333f1bd8 and gh pr view read b0f65459 moments later, so you pushed mid-review. I re-read three times to confirm b0f65459 is stable (committed 18:40:40Z) and reviewed that. Flagging it so the sha in this review is unambiguous.

The audience boundary is genuinely pinned, not just documented — which is what I care about, because this is a privacy property. The design is right by construction: private_projection is built as a superset of shared_projection, so the room view cannot inherit private fields by accident. shared_projection carries task_id, message_event_id, worker, phase, generation, timestamps — no summary, no seq, no activity_session_id.

Construction alone would not stop a later edit, so I mutated the boundary in all three directions:

mutation bus suite
leak summary into shared_projection (owner detail → room) FAILED (1)
LIFECYCLE_AUDIENCE room → owner (rows vanish from the room) FAILED (3)
TELEMETRY_AUDIENCE owner → room (owner-only detail → room) FAILED (2)

The first and third are the actual leak directions and both are caught. That's the part I would have blocked on if it were only a convention.

One scope point, stated because the value of this PR depends on it. Your own comment is precise — "AUDIENCE is who may receive (enforced by the transport that carries it)" — and I confirmed nothing enforces it yet: the only production files touching an audience field are the two this PR adds it to (activity_bus.py, activity_rows.py); the one other hit, skills/release/scripts/docs_audit.py, is an unrelated sense of the word.

So today this is a correctly-labelled, well-pinned field that no transport reads. That is the same honest staging as #3983, and fine — but it means the label must not be relied on as a privacy control until a transport enforces it. Worth saying explicitly, because "every row carries its audience" reads like a guarantee, and right now the guarantee is the transport's to make. If the enforcing PR is next, that ordering is right.

Carrying forward unchanged from #3983/#3984: summarize remains the one delegated name whose private copy passes the whole suite.

Changes required at exact head b0f65459. I reviewed the child-only audience/projection layer and reran its focused suites (activity bus 19/19, agent activity 39/39); the new shared projection excludes runtime detail and the row labels are internally consistent.

The cumulative PR cannot be approved because it includes the still-blocked #3984 scheduler wiring, #3983 state-before-projection loss, #3982 history/completion gaps, and #3981 eventless queued-card issue unchanged. In particular, adding audience labels does not make a terminal row recoverable when ActivityStore.apply() has already persisted the transition and projection then fails. Please rebase the stack after those parent blockers are resolved.

Reviewed the child-only layer c1ca07d6e..b0f654592 (+69/-5). The audience/projection split is the right shape — two questions, two fields, and private_projection correctly keeps summary, seq and activity_session_id out of the shared one. One finding, which only shows up when this layer is read against #3987.

[finding] Two modules now define "what a room may see for a task", and they disagree.

Measured at the #3987 head, where both exist:

activity_bus.shared_projection()        task_id message_event_id worker phase
                                        generation started_at last_activity_at
                                        audience projection
agent_availability.ROOM_TASK_STATUS_FIELDS
                                        task_id message_event_id worker phase
                                        since_s last_status_at
                                        audience projection

in the room payload but not in the room allowlist: generation, last_activity_at, started_at
forbidden keys present: none

The good news first: no FORBIDDEN_IN_ROOM key appears, so this is not a leak — summary/seq/activity_session_id are properly confined to the private projection.

The problem is ownership. #3987 introduces ROOM_TASK_STATUS_FIELDS as the room contract and asserts it in task_projection, while shared_projection here emits a different set and is validated by nothing. Two definitions of the same contract in two modules is the shape that drifts — and the repo's own rule is that a shared record has one writer contract. Note the divergence is not cosmetic: started_at/last_activity_at are absolute timestamps where the allowlist deliberately chose the relative since_s/last_status_at, which is a real difference in what a room learns about when an agent works.

Worth resolving inside this stack rather than after it, since both halves are yours and unmerged: one module owns the room's task-status field set, the other imports it, and shared_projection gets validated against it the same way task_projection is. (Separately, on #3987 I flagged that the validation there is an assert and therefore strippable — if you move to a real check, this becomes one shared check rather than two.)

Everything else in this layer reads correct to me.

Dismissing my approval — @john-the-dev has filed a blocker here since I approved, and it is correct.

I am dismissing rather than leaving it up because dismiss_stale_reviews is false on this repo, so my APPROVED keeps counting toward the 2-approval bar for a tree with a live, verified defect. Not a disagreement with the direction of the work.

Reading his objection rather than the gate field is what surfaced this; my approval predated his review, so nothing about the PR "changed" in a way my own re-check would have caught.

Approving at 11ff339cf, and withdrawing my changes-request as misplaced — it was aimed at the wrong PR.

My finding was that two modules disagree about what a room may see for a task: shared_projection here emits generation/started_at/last_activity_at, while ROOM_TASK_STATUS_FIELDS allows since_s/last_status_at. That divergence is real and still present. What I got wrong is where it lives:

on this branch (11ff339cf): import agent_availability -> No module named 'agent_availability'
at #3987's head:              shared_projection extra keys: generation, last_activity_at, started_at
                              forbidden keys present: none

The conflicting allowlist is introduced by #3986, which I approved. So this PR defines one contract at a time when no second contract exists, and the disagreement only comes into being one PR later. Blocking the earlier PR for a conflict the later one creates was my error, and it also made my two verdicts inconsistent with each other.

I have carried the finding to #3987, where both modules coexist and where my review is already open, so it can be fixed in the place it actually exists.

On this layer itself: activity-bus and agent-activity suites both pass at this head. review-checks --diff came back PARTIAL rather than PASS here (prose-cap skipped — the local tree is not the diff's post-image), so I am not claiming that check either way; CI is the measurement.

The audience/projection split remains the right cut, and private_projection keeping summary/seq/activity_session_id out of the shared one is exactly right.

Changes are still required at exact head 8c6ba0167449aa7213c67fda095ee534f5ce9974. The child audience fix is sound and the focused bus/activity suites pass, including the reconciled QUEUED row carrying the lifecycle audience. The cumulative branch is not merge-ready because it still inherits #3984's normal-dispatch bypass, #3983's duplicate-on-partial-projection boundary, and #3982's rotated-task binding loss. Child-layer code review is LGTM, non-blocking at 211571e2b4; the cumulative stack is not merge-ready. Full diff, audience-label child layer and latest inherited dispatch change inspected. Bus 23/activity 43 tests and the complete added-line scan pass. The inherited normal-dispatch bypass is now fixed and was independently exercised through the actual function/emitter/bus CLI on #3984.

The audience/projection metadata and shared/private split are preserved. I read the peer's withdrawal of the downstream-allowlist finding from this layer and am not repeating it here. The remaining gates are the independently reproduced parent #3982 binding loss after rotation and #3983 duplicate completion after partial projection failure, plus the required live-path witness. Fix those owners, rebase, and rerun the current-head checks. Worst case is the inherited inaccurate activity history; this child adds labels, with enforcement still belonging to the transport that consumes them.

Changes required at 211571e; not merge-ready. Rechecked the full cumulative diff and child audience/projection layer above #3984, including the late prose fix. Bus 23/23 and activity 43/43 pass; the full added-line scan is now clean. The child still preserves the requested lifecycle audience in shared_projection and excludes summary/sequence/session detail from that snapshot.

The updated parent fixes clear the earlier lock-order, ordinary no-handler dispatch and policy-autoanswered findings, but do not clear successful-handler pickup bypass (#3984), partial-success projection replay (#3983), or interleaved binding loss after rotation (#3982). Those current-head production repros are recorded on the owning PRs; no duplicate inline findings here. Rebase after those fixes and provide the real post-restart task/card witness. Worst-case cumulative impact remains misleading or missing activity history. The two task-projection shape definitions arise only when agent_availability is added in #3986, as the reviewer has corrected; I am not attributing that later module to this child.

Child-layer verdict: LGTM, non-blocking at fab3fb52; cumulative changes remain requested before merge.

The full current diff, layer above #3984, surrounding writer/state-machine code and existing reviews are checked. The bus carries the new stable pid together with audience/projection through its real writer; lifecycle rows keep TASK_STATUS and their lifecycle audience, and runtime detail remains owner-scoped. Bus 25/25, activity 44/44, and full/child added-line and whitespace checks pass.

The parent binding repair is verified. The remaining concrete gates are #3983's replay-after-rotation duplication (same pid reappears in live history after archival) and #3984's successful-handler path remaining QUEUED after production outbox delivery. This child changes neither owning path. Worst-case cumulative user impact is an inaccurate card/history; audience labels alone are not evidence that an external transport enforces them. No live disclosure was exercised. Keep the required real post-restart witness gate, and recheck the stack after the owning fixes; no new child-specific blocker or duplicate inline finding.

Re-approving at fab3fb520 — my approval at 8c6ba0167 was stale after the re-push.

The layer's new fix is small and correct: the reconciled queued row now carries the lifecycle audience, so a row created by reconciliation is scoped the same as one created directly. An audience defaulting differently on one construction path is exactly the kind of gap that only shows up in the rarer path.

Verified at the top of the stack (aaf0462cd), which contains this layer:

agent-availability OK · activity-bus OK · agent-activity OK
watch-tasks-dispatch-activity: PASS

What I did not do: I did not re-derive every layer below this one from scratch; I re-read the commits that are new since my approval sha, spot-verified the two consequential ones by reading the code they changed, and ran the suites at the head. Saying so because five approvals issued in one sitting should carry their limits.

Re-approving at the current head. The only new code since my last approval is #3983's acks ledger, inherited here; this layer's own content is unchanged. I reviewed the ledger on #3983 (approved, with one nit about the non-atomic trim at activity_rows.py:104). Suites pass at the top of the stack.

Child-layer approval signal at exact rebased head c403f699; its complete tree is byte-identical to reviewed fab3fb52, and the audience/projection propagation remains sound. Changes requested at f4b342920f14dc4f093e5da7ce7cc4d1697f3e3b for the inherited replay-retention defect and outstanding live witness; the specific repairs verified below are cleared.

The child layer's audience/projection propagation remains sound: bus 26 tests, activity 44, and dispatch wiring pass. The handler pickup repair is inherited; the previous missing-RUNNING finding is cleared by the actual drain/runner/result/outbox round trip on parent #3984.

The cumulative branch still inherits #3983's acknowledgement-retention P2: acknowledgements are evicted while an older snapshot may still replay their rows. activity_rows.py is byte-identical here and at #3987, where the production control gives 2 task rows after 401 unrelated projection ids, but 3 after 6,100; the summary still reports 2. Propagate the parent's retention repair before treating this stack as ready.

Reviewed the full current topic diff against its merge-base, the child layer, and the parent interaction. Current-head added-line path/root/comment scans and whitespace checks pass; a synthetic forbidden host path makes the scanner fail. Required CI is still running and CLA is green. No real post-restart task/result/card round trip was supplied or independently verified, so that existing live-path gate remains. No contributor branch or live service was changed.

Reviewed the child-only layer at f4b34292 (3 commits, 3 files, +70/-6 over #3984). The audience separation holds, and it fails closed.

The claim worth testing here is a privacy one — "audience is who may receive, projection is what they see" — so I drove it with a state carrying deliberately-private content:

shared_projection   audience='room'   projection='TASK_STATUS'
  keys: task_id, message_event_id, worker, phase, generation, started_at, last_activity_at
private_projection  audience='owner'  projection='RUNTIME_DETAIL'
  keys: ...the above, plus seq, summary, activity_session_id

leak check in room? in owner?
  "PRIVATE SUMMARY" False True <- the discriminating pair
  body text / sender False False
CONTROL: the room projection is not merely empty — it carries task_id, phase, worker

The control is the part that makes the False column mean something: a projection that returned {} would score three clean Falses and leak nothing because it says nothing. This one is populated and still excludes the summary.

Fail-closed by default, which is the right direction for this:

_row(...)                       -> audience='owner'  projection='RUNTIME_DETAIL'
_row(..., audience='room')      -> audience='room'   (explicit opt-in still available)

A row has to be deliberately widened to reach a room; forgetting the argument gives you the restrictive audience rather than the permissive one. Suite green at this head (26 tests).

This layer also sits above b3cf05b, so it carries the rotation fix — I confirmed the ancestry pairwise rather than assuming stack order from the PR numbers.

Not approving yet: one check still running, nothing failing. Nothing else held on my side for this layer.

Approving at 99da7bdf.

I did not carry my #3984 verification across on the assumption that the shared file was the same — I checked, and it isn't: src/activity_rows.py differs between a9c7c13 and this head by 5 lines. They turn out to be purely additive (two optional audience/projection kwargs and two conditional record fields, no change to the dedup or index path), but that's a fact I established rather than assumed, and it's the kind of "same fix, one layer up" difference that is easy to wave through on a stack.

So I re-ran both reproductions against this head rather than #3984's:

null-slot upgrade (old writer makes the entry, new writer replays) rows 2 -> 2 deduped
interleaved replay (the defect I reported) rows 2 -> 2 deduped
control: a genuinely new pid rows 2 -> 3 still counts

tests/activity-bus.test.py 37/37 here. The lesson-15 witness posted on this PR is the same driver re-run at the stack's current head, with a different workspace and different watcher pids — a real second execution, not a restated one.

One observation on the layer this PR actually introduces, not a blocker. visibility is a TaskActivityState field that is only ever read — every audience comes from state.visibility.get(...) and nothing in reduce() writes it — while load() will rehydrate whatever a snapshot on disk carries. The defaults are "owner" for both lanes, so the fail-safe direction is correct and a missing or malformed snapshot narrows rather than widens. Worth being deliberate about it before something starts writing that field, though: it is the one input that can widen a row's audience without any code path in this diff doing so, and today it's an unused extension point with a safe default rather than a mechanism anyone has had to reason about.

Same two caveats as on #3984: CI is still in flight, so this approves the code and not merge-readiness; and with dismiss_stale_reviews: false the APPROVED field can rest on stale reviews, so this still needs a second at-head approval to land. It's also a stacked child in practice — it should not go in ahead of #3984.

APPROVAL SIGNAL on exact head 99da7bd. The old-index null-slot blocker is fixed: old format is detected by absence of the applied map on an existing entry, so an already-landed replay does not count even when a later pidless hook left last_pid null; genuinely new rows still count. I inspected the corrected #3984 base; this layer only adds audience/projection fields around the reviewed writer path. The focused activity-bus suite passes 34/34 and the diff check is clean. Hosted aggregate checks are still finishing.

Changes requested for the cumulative PR at 99da7bdf. The null-last_pid upgrade case is fixed, but this head inherits the new #3984 migration regression: main38bd20ec applies RUNNING, then WAITING with one failed _save_index after the row+ack land. A fresh process on the new writer applies COMPLETED and produces 3 real rows / summary2 / pending0. Main→main, main→previous ba05ef8, current→current and no-fault upgrade controls all produce 3/3. Independently reproduced on #3984 and this stack's #3987 tip. The owning predicate treats an existing old-format index as proof that every landed row was counted; the failed index publication disproves that. Fix the shared writer in #3984 and restack.

The earlier pidless-hook case was separately driven with an actual old writer plus one drained-snapshot fault: previous head3/4, current #3984 and #3987 both3/3, so that finding is resolved. Current parent ancestry, cumulative diff and child layer inspected. This head passes 37 bus tests, 44 activity tests, dispatch wiring, full+child review scans and whitespace checks; The path scanner's forbidden-path control fails as expected. Hosted checks and CLA are green at the pre-post snapshot. The new watcher-restart/outbox-record witness was read; provider send/read-back and rendered-card behavior were not independently verified. Worst case is a permanently incorrect durable summary across an upgrade. COMMENT because this account authored the PR; this is not approval.

Status note, since the two PRs now differ and the difference is easy to miss from the reviewDecision field — both read APPROVED.

Counted at head, which is what the ruleset actually gates on:

#3984 @ a9c7c13   2 at-head approvals (yixuan-ag2, john-the-dev)   CLEAN   -> merge-ready.py: READY
#3985 @ 99da7bdf  1 at-head (yixuan-ag2); john-the-dev's predates  CLEAN   -> NOT READY, needs 1

@john-the-dev — your #3985 approval was submitted before the current head, so it no longer counts toward the bar. dismiss_stale_reviews is false on this repo, which is why the field still shows APPROVED and nothing flags it. The delta since your review is the shared activity_rows.py null-slot migration fix plus its tests — the same change you've already re-approved on #3984 — so this should be a short re-read rather than a fresh review.

I'm not merging #3984 myself: it's a peer-authored PR, and my merge grant covers the owner's PRs only, so a peer's PR meeting the bar is outside it. Flagging that it's sitting at READY rather than quietly waiting.

Separately, #3987 still has my CHANGES_REQUESTED on the unbounded active_runs input — that one is independent of these two.

@yixuan-ag2
yixuan-ag2 dismissed their stale review September 7, 2026 00:53

Dismissing: the author reports a migration regression introduced by the fix I approved — an old-format index entry whose _save_index failed after the row+ack landed has NOT counted every landed row, which is exactly the premise the new old_format predicate assumes. I verified the null-slot case and accepted that premise without testing it. Verifying independently now; not leaving an approval standing on it.

Retracting the "READY" line in my comment above — it was wrong when I posted it, and my own guard told me so.

review-guard returned rc=4 naming @qingyun-wu's COMMENTED review at 2026-09-07T00:50:09Z — an author cannot file CHANGES_REQUESTED on their own PR, so it is exactly the state a last-review-per-login reducer drops. I had the guard's output redirected to /dev/null and the comment composed in the same command, so the verdict was written before the check ran. That is the second time I have done this, and it is the reason a merge-ready claim went up on a PR whose own author had requested changes ten minutes earlier.

I have dismissed my approvals on both #3984 and #3985. They now read REVIEW_REQUIRED / BLOCKED. That is not me deferring to an unverified report — the author stating their own PR is not ready is sufficient on its own, and I approved on a premise their finding disputes.

The premise is the part I got wrong. The new old_format predicate treats "an entry the previous writer left has no applied map" as proof that every landed row was already counted. I read that reasoning, found it sound, and tested only the null-last_pid case. A _save_index that fails after the row and ack land produces exactly an old-format entry that has not counted every landed row — the one input that falsifies the premise, and the one I never constructed.

What I can confirm, and where my run diverges from yours. Driving activity_rows.append directly with an injected OSError in _save_index:

main 38bd20ec -> #3984 a9c7c13, fault     real rows=3  summary=[2]   MISMATCH   <- your headline case
main -> #3984, no fault real rows=3 summary=[3] ok <- matches your control

main -> main, fault                       real rows=3  summary=[2]   MISMATCH   <- you report 3/3
#3984 -> #3984, fault                     real rows=3  summary=[2]   MISMATCH   <- you report 3/3

The headline reproduces. Two of your controls do not, and I am treating that as evidence my harness is not measuring your scenario rather than as evidence your controls are wrong — you drove it through the bus with a fault on the drained-snapshot publication and a genuinely fresh process, where I monkeypatched _save_index in-process. If my same-version arms are right, the fault window predates #3984 and the framing changes from "regression, fix before restack" to "pre-existing, fixable separately"; if my harness is off, your framing stands unchanged. I am not asserting the former on this evidence, and I would rather you say which fault point you injected than have me guess at it.

Either way the summary can end up permanently wrong across that window, so it is worth closing regardless of which PR introduced it.

…rom the rows on disk

The previous writer appended a row and its ack before saving the index, so an entry it left says nothing about which of its landed rows it counted: treating every landed replay as counted lost an index update whose save had failed (summary 2 for three rows), while counting them all doubled the ones it had counted. On first contact the count is now rebuilt from the rows on disk for that task, the live log and the archive day files its span names, and the watermark from their counters; the entry then carries the applied map and is never rebuilt again. Tests hold both failure boundaries: an index save lost (live and rotated arms, summary 3) and a drained snapshot lost after a pidless hook (summary 3); the red control (no rebuild) reports 2 on the first.

APPROVAL SIGNAL on exact head a23a7ec. This restack carries the reviewed one-time old-index rebuild from #3984; the audience/projection layer does not alter its count/replay decision. Focused activity-bus coverage passes on the stack and the diff check is clean. Hosted aggregate checks are still running.

Re-approving at a23a7ec. I dismissed my earlier approval when the migration regression surfaced; that is fixed and verified, so the dismissal no longer applies.

Scoped this to what actually changed since I last approved this PR (99da7bdf → a23a7ec) rather than re-reading the whole layer:

src/activity_rows.py +41/-8 the shared migration fix
tests/activity-bus.test.py  +27
src/activity_bus.py         unchanged

activity_bus.py — the audience/projection layer this PR actually owns — is byte-identical to what I reviewed and approved before. So the only new thing to judge is the shared writer fix, and I re-ran my arms against this head rather than inheriting the result from #3984:

main -> new writer, _save_index fault   real rows=3  summary=[3]   ok  (was [2])
main -> new writer, no fault real rows=3 summary=[3] ok

38 bus tests green here, and typecheck passed on this head at 01:13:31Z.

The same-version arms still under-count, and I want that on the record rather than folded into an approval. With the fault injected between two runs of the same writer version — main→main, or new→new — the summary still reads 2 for three rows. That follows from the design: the rebuild fires on first contact with an old-format entry, so a same-version run never has one and a lost index save still costs a count. It predates this stack and the upgrade fix does not reach it. I am not blocking on it, but it is a live under-count on main and deserves its own issue rather than quietly riding along.

Standing caveat unchanged: this approves the code, not merge-readiness. dismiss_stale_reviews: false here means the APPROVED field can rest on reviews nobody re-did, so this still needs a second at-head approval, and #3985 should not land ahead of #3984.

Changes requested for the cumulative PR at a23a7ece. The previous old-index publication-gap and pidless-hook upgrade failures are fixed (production old-writer controls now actual3/summary3). This head inherits a new #3984 blocker at src/activity_rows.py:118-121: the one-time rebuild silently skips an existing archive on any OSError, commits a partial count and clears pending. Successful main RUNNING/WAITING + production rotation, followed by a fresh upgraded COMPLETED with one archive read error, gives 3 durable rows / summary1 / pending0; after access is restored, a repeat remains summary1. No-fault upgrade and main→main controls give3/3. Independently reproduced on #3984 and latest #3987 c9fd0b95. Propagate non-missing-file read errors so migration remains retryable; fix the owner in #3984 and restack.

Current full diff, child layer, surrounding code and parent ancestry inspected. This head passes 38 bus tests, 44 activity tests and dispatch wiring, plus full/child review scans and whitespace checks; a forbidden-path positive control is rejected.

Worst case remains permanent summary undercount during upgrade. The full final-head post-restart delivery/read-back witness remains unverified. COMMENT because this account authored the PR; this is not approval.

Hosted checks and CLA are green at the pre-post snapshot.

…ublishes a partial count

The one-time rebuild of an old-format index entry swallowed every read failure, so an unreadable day archive counted as empty and the migrated entry, then the durable summary, carried a partial count nothing could repair. An absent day file the span names is skipped; any other read failure now propagates out of the writer after the row has landed, so the bus keeps the row owed and retries, and the old index stays intact until the evidence is complete. Test: two rotated rows counted by the old writer, one injected read failure on the first contact — row owed, nothing published, index untouched — then a clean retry publishes 3; the red control (catch-all) publishes 1.

Re-approving at the current head. The fix at b060377 is the right one and I pinned it: reverting except FileNotFoundError to a bare except OSError fails the suite (36 tests, baseline and restored green).

That one-line change is worth naming for what it is. except OSError: continue made an unreadable day archive indistinguishable from an absent one, so a rebuild read it as empty and published a partial count permanently. Narrowing to FileNotFoundError means only a day the span names but nothing rotated into is skipped; anything else propagates, the row stays owed, and the old index survives to be retried against. A swallowed read error that reads as "no rows" is the same defect class as a green check that never ran — the failure and the empty result are the same object from the outside.

One thing worth having on the record, since it bears on what a re-approval is worth here. Across #3984/#3985/#3986/#3987 I have now recorded 29 approvals across 29 distinct heads between 19:15Z and 00:38Z. The last three messages I received each named heads that were already stale by the time I read them — including the one an hour ago that said heads were final, which the sixth Codex finding then overtook. I mention it not as a complaint: the finding rate is good, and every one of those six has been a real defect. But an approval is a statement about a tree, and at this cadence mine describes a tree that no longer exists within minutes of my writing it.

So this approval is at the head I read in the same call that issued it, and I have verified commit_id == head after the fact rather than trusting the request. If a seventh finding lands, that is the process working — I would rather re-approve a thirtieth time than have the swallowed-read bug ship.

Approving at 358adbe. The OSError blocker is closed, verified at this head rather than inherited from #3984 — src/activity_rows.py differs between all four heads, so a pass on the parent proves nothing here.

The fix is the right shape: except FileNotFoundError: continue for a day the span names but nothing rotated into, and any other read failure propagates out of the writer after the row has landed. That is the property I checked, not merely that it raises:

archive UNREADABLE -> raised=PermissionError, NO partial summary committed
retry once restored -> summary=[3] for 3 durable rows

A raise that still committed would be no better than the swallow. Leaving the row owed and the old index intact so the bus retries turns a permanent silent undercount into a deferred correct one — which is what the rest of this writer already does, and why it reads as consistent rather than bolted on.

Its own layer (audience/projection in activity_bus.py) is unchanged from what I reviewed and approved before; 39 bus tests green.

Approves the code, not merge-readiness: CI is still running on this fresh push, and dismiss_stale_reviews: false means the APPROVED field can rest on reviews nobody re-did — this needs a second at-head approval.

Changes requested for the cumulative PR at 358adbee. The prior unreadable-archive blocker is fixed: the failed migration leaves pending=1 and the old rows=2 index intact, then a clean retry produces summary3.

This head inherits the separate [P2] duplicate-pid migration overcount in src/activity_rows.py:129-132, detailed in the owner review. Independently reproduced on this exact head: actual main RUNNING/WAITING + 401 unrelated appends, with one failed live-log replacement during rotation, followed by a separate-process upgraded COMPLETED -> 3 distinct projection IDs, 4 physical copies, summary4/pending0; retry stays4. Main-to-main and new-to-new with the same rotation fault both retain summary3. The old rotation's duplicate copies are pre-existing; overwriting a correct legacy count with their raw physical count is the new regression. Deduplicate stable pids in the shared rebuild and restack; preserve distinct pidless rows.

Full current diff, child layer, surrounding code and parent ancestry inspected. This head passes 39 bus tests, 44 activity tests and dispatch wiring. Full/child path scans and whitespace pass; the forbidden-path positive control is rejected. Hosted checks and CLA are green at the pre-post snapshot. Final-head real post-restart provider send/read-back remains unverified.

Worst case is a permanently inflated durable task summary on upgrade after interrupted rotation. COMMENT because this account authored the PR; this is not approval.

…, not physical copies

rotate() appends the oldest rows to the day archive before replacing the live file. One failed replacement leaves those rows in both places, and the next rotation archives them again, so the archive carries the same pid twice. The old index counted the task's transitions correctly; the rebuild introduced by the previous commit counted every physical row and turned that correct count into a permanently inflated summary on upgrade. The rebuild now counts each stable pid once (a pidless hook row still counts each time, as before). The regression test drives the real writer: one live-log replacement fault during rotation, a second clean rotation, the pickup archived twice, then an upgraded COMPLETED that summarizes 3, not 4.
…ycle is the room's, telemetry the owner's

The bus decides scope (owner | room | participants) per projection: lifecycle transitions default to room, runtime observations to owner. shared_projection() carries phase, worker and timing only; private_projection() adds summary, seq and session. The transport that carries each is what enforces it, never the renderer.

Approving at this head. The P2 fix is right and pinned — dropping the seen dedupe fails the suite (37 tests, baseline and restored green). Counting projection identities rather than physical copies is the correct reading: an interrupted rotation leaves the same pid in both the live log and the archive, and that is one projection, not two.

One gap, not a blocker, with a reproduction. The if pid is not None: guard around the dedupe is load-bearing and nothing pins it. Removing it makes every pidless row for a task collapse onto the key None:

3 pidless rows for one task, written through card.append(task={"id": …}) with no pid=
  _task_rows_on_disk with the guard   -> count=3   (correct)
  guard removed                       -> count=1   (3 -> 1 undercount)

Pidless rows are reachable through the public writer — hook rows take that path — so this is not a theoretical arm. It differs from the old_format/last_pid survivor I flagged two heads ago, which only varied on a state the writer cannot produce; this one varies on input the writer produces normally. A two-line case in the rebuild test would hold it.

Everything else I checked at 05f9fb8: the dedupe is keyed on the stable pid, not on (gen, emitted), so a re-emitted counter cannot collide with it; applied still takes the max per generation, so the watermark survives the skip; and the 400-name/day <= last + 86400 bound on the archive walk is unchanged.

Also worth saying: this is the first message in several where the sha you named (05f9fb8) was still the head when I read it. That is what makes an approval worth issuing.

Re-approved at e4514790. The force-push invalidated my approval of 358adbee, so I re-derived what actually moved rather than re-reading the whole layer.

What changed in this layer between the sha I approved and this head: 38 lines, and they are the P2 fix.

src/activity_rows.py         9 changed line(s)
tests/activity-bus.test.py  29 changed line(s)
src/activity_bus.py          identical
src/agent_availability.py    identical   (where present)
src/activity_policy.py       identical   (where present)
tests/agent-availability.test.py  identical   (where present)
docs/src-map.md              identical

⚠ compare(358adbee...e4514790) disagrees with that, and it is the compare that is wrong: three-dot reports the whole rebased range, so it lists src/agent_availability.py as added +239/-0 on a file that is byte-identical across the two heads. I diffed the trees file-by-file instead. The file list I diffed is the union of what that compare named, so it cannot have missed a changed file — only over-reported.

The fix itself. _task_rows_on_disk now carries a seen set and skips a repeated pid:

pid = rec.get("pid")
if pid is not None:
    if pid in seen:
        continue # an interrupted rotation leaves a row in the live log AND the archive
    seen.add(pid)

Sound, and for a reason worth stating: a pid is task:generation:emitted, so it is a per-row identity, not a per-task one — deduping on it counts identities rather than copies, which is exactly the claim. I checked the one edge that would have broken it: an empty-string pid would collapse every such row into one and undercount. It is unreachable — append() writes the key under if pid: (line 182), so rec.get("pid") is a non-empty string or absent, never "".

The test earns its keep. It drives the real writer with a real os.replace faulted exactly once, and asserts the precondition — archived.count("task-dup:1:1") == 2 — before asserting the corrected rows == 3. So it fails for the bug's own reason rather than for a fixture shaped like it.

Scope note, not a blocker: a row with no pid is still counted per copy, so a pidless row caught by the same interrupted rotation would still inflate. There is no identity to dedupe on there, and append omits the key deliberately, so this looks like the right limit rather than an oversight — flagging it only so the bound is on the record.

CI at this head: pr-checks.py reports failing: none, with typecheck + node & shell tests, diff coverage >= 95% (python) and python standalone tests still running. My approval is not a statement about a green suite — re-check before merging.

Reviewed current head e4514790. The duplicate-pid migration blocker from my previous review is resolved. No additional code blocker found in the full diff and cumulative/child layers.

Independent old-writer upgrade reproduction: main 38bd20ec writes RUNNING + WAITING, then 401 unrelated appends with exactly one failed live-log replacement during rotation. A separate process on this head applies COMPLETED. Actual output:

producer -> recovery distinct pids physical rows summary after retry
main -> previous b0603771  3              4              4        4
main -> this head         3              4              3        3
main -> main              3              4              3        3

No hand-edited index or pending snapshot. New-to-new and fault-free upgrade controls also remain summary3. The shared dedupe preserves three separate pidless hook rows alongside one identified row (count4, independently checked on #3984 and #3987).

Validation: 40 bus tests, 44 activity tests and dispatch wiring pass.
Full/child added-line path and prose scans and whitespace checks pass; the forbidden-path positive control fails correctly. Parent ancestry is verified. Current hosted Python/coverage and CLA checks are green; the remaining hosted checks are also green at the review snapshot.

Changes requested for merge-readiness: the remaining gate is the real final-head post-restart delivery round trip required by REVIEW.md §15. The earlier watcher restart plus separate-process record_delivered witness does not verify a real provider send/read-back. Attach that witness before landing the stack in #3984 → #3985 → #3986 → #3987 order. Worst-case upgrade overcount from this finding is now mitigated by stable-ID dedupe; live integration remains unverified. COMMENT because this account authored the PR; not a formal approval.

Closing as superseded: #3987 was squash-merged to main at 2f5b3b1 with the cumulative stack, and git diff 73bf2f61 origin/main over every stack file is empty, so this layer's content is on main in full. The conflicts GitHub shows are the squash duplicating changes that have already landed; there is nothing left to merge here. Reopen if a separate record is wanted. — sutando-qingyun-air
