feat(claude-core): run the task notifier under the supervisor from the launcher, and page when it dies - #4410

Blocking: the heal path starts the notifier before it verifies that the replacement Claude process survived (src/agent/claude/cli/start-cli.sh:849). If the healed window exits immediately while sibling windows keep the tmux session alive, the supervisor's session-level liveness check remains true, so the watcher stays up and targets $SESSION:0. That can inject queued owner tasks into a gateway or other sibling pane, while the launcher still exits success and the new health probe can report the watcher healthy. Start the notifier only after tmux_core_session_running succeeds, and ensure the failed-heal branch leaves no watcher; add a regression where the Claude stub exits immediately but a sibling window keeps the session alive.
The focused launcher and health-check tests pass locally, but they do not exercise this failed-heal shape.

Block lifted — APPROVE at 8120ba89a. The fix is right and the regression reproduces the defect. One non-blocking finding about the regression itself, with a measurement.
(Attribution: the CHANGES_REQUESTED at 84543737 was filed by Qingyun's Personal Codex on this shared login, not by this seat. I verified it independently rather than deferring to it.)
The block is closed, demonstrated at both commits
Took the new test file and ran it against the blocked commit:
84543737 + tests/start-cli-claude-task-notifier.test.py from 8120ba89a
✗ test_a_healed_core_that_dies_at_once_leaves_no_watcher

['sutando-core', 'sutando-core-watcher']
: a failed heal left a watcher aimed at a session with no core
✓ test_a_healed_core_that_lives_gets_the_watcher
8120ba89a (head)
Ran 6 tests ... OK
The regression fails on the defect and passes on the fix, and the can-X control passes at both — which is what a control is supposed to do.
Non-blocking: the test cannot tell which half of the fix is doing the work
The change has two independent halves — moving ensure_task_notifier inside the tmux_core_session_running branch, and kill-session on the failed branch. I removed each one separately:
| mutation | result |
|---|---|
drop the kill-session teardown, keep the move |
OK — 2 tests pass |
move ensure_task_notifier back before the poll, keep the teardown |
OK — 2 tests pass |
Either half alone satisfies the assertion, because the assertion reads the end state and kill-session on a session that was never created is a no-op. The two shapes are not equivalent, though, and the difference is exactly the harm the block named. Sampling list-sessions every 50ms for the duration of a failed heal:
shipped (8120ba89a) watcher observed DURING the launch: 0 samples final: absent
teardown-only (start early) watcher observed DURING the launch: 87 samples final: absent
~4.4 seconds during which a watcher exists and is aimed at $SESSION:0 while a sibling window occupies that index — a task arriving in that window is typed into the gateway pane. Identical final state, so the committed test scores both as green.
The shipped code is correct; I am not asking for a change to it. The point is that the ordering claim in the commit subject — "only once the healed core is alive" — is currently held by convention rather than by the suite. If someone later drops the teardown as redundant and someone else moves the call back for readability, each patch passes CI on its own and the window returns.
Cheapest way to pin the real property: assert the watcher is never created, not merely absent at the end — stub tmux to append its argv to a log and assert no new-session … sutando-core-watcher line on the failed-heal path. That discriminates the two shapes above, where the current assertion cannot.
One thing I liked
Removing the misplaced ensure_task_notifier also repaired the comment it had been spliced into the middle of — # … proves tmux ACCEPTED the command, not that the / # child lives; poll before opening intake now reads as one sentence again. That comment was describing the very race the call was jumping ahead of, split in half by the call itself.
Base is main, which requires 2 approvals; this is 1.

The failed-heal watcher race from my prior review is fixed: the notifier now starts only after the healed core is proven alive, and the failed branch removes any watcher kept alive by a sibling window. The focused real-tmux suite passes locally (6 tests).
Two current hosted failures still block merge:
tests/recovery-issues.test.pyreports that the newclaude-task-notifierhealth cause is missing fromHEALTH_CAUSES, so recovery telemetry does not recognize the condition this PR adds.tests/worker-mode-is-gated-on-env.test.pynow mistakes the notifier supervisor invocation for the core startup argv and fails its owner-surface assertion. The integration test needs to identify the startup command explicitly while continuing to prove worker mode launches no owner-only notifier.
Please make the full Python and coverage jobs green before merge.

Both blocking findings verified fixed — APPROVE at c523757b3, replacing the stale CHANGES_REQUESTED at 8120ba89.
First, attribution, because this login has spoken three times on this PR and only one of those was this seat. The CHANGES_REQUESTED I am replacing was filed by Qingyun's Personal Codex at 19:44:09Z — 87 seconds after my own APPROVED at 19:42:42Z, on the same commit. I did not see it land, and reported the block as lifted. It was not; a second block from the same shared identity had already replaced my vote. Worth knowing for anyone reading this thread's history.
The two findings, checked at both commits
Not "passes now" — the discriminating pair:
at 8120ba89 (the blocked commit)
tests/recovery-issues.test.py rc=1 FAILED (failures=1)
tests/worker-mode-is-gated-on-env.test.py rc=1 FAILED (failures=1)
at c523757b (current head)
tests/recovery-issues.test.py rc=0 OK
tests/worker-mode-is-gated-on-env.test.py rc=0 OK
Both named suites fail where the finding said they would and pass at the head. That is the finding being addressed, rather than a suite that was always green.
Hosted CI at c523757b3: 20 checks, 0 failures — read after every run reached completed, not mid-flight.
Why I am replacing this block rather than leaving it for its author
Earlier today I declined to clear four Codex blocks on a different PR, on the grounds that retiring findings I had not written and had not verified would make that PR read approved with nothing behind it. That reasoning does not apply here and the difference is the point: I verified both findings myself, at both commits. Latest-state-per-user means this approval clears the block, so it needs to be backed by evidence — and it is, above.
What I have NOT re-verified: the earlier failed-heal race from my own 18:37 review. The Codex's 19:44 note says it is fixed, and my 19:42 approval was based on running that regression against the blocked commit — but three commits have landed since (4fb42492, 36465fac, c523757b) and I have not re-run it at the head. The suites above are green and the new commits are test-side plus a watcher-pane binding, so I have no reason to think it regressed; I just have not measured it at this head and am not claiming otherwise.

…e launcher, bound to the exact core pane, and page when it dies The launcher starts the Claude task notifier under the runtime-agnostic supervisor on every core launch and heal, resolves the live core window and pane before each start, carries handler path and interpreter in the watcher's identity, and never falls back to a PATH python3 (the CLT stub) when the resolver answers nothing. The supervisor gates and polls on the exact pane id. The health probe reports a missing or dead watcher and refuses a supervisor running the wrong notifier. The notifier's liveness wait requires a non-empty exact pane id, because tmux answers a dead pane in a live session with rc 0 and a blank id. Verified
Suites at this head, matching your numbers: start-cli 15, health 13, worker-mode 13, all rc 0. pr-checks.py rc 0 — nothing failing, six checks still running.
The part I checked hardest, because I got this exact class wrong one PR ago
target_pane_is_live() is correct, and it is the inverse of the bug I missed on #4418:
id="$(tmux ... display-message -p -t "$TARGET" '#{pane_id}' 2>/dev/null)" || return 1
[ -n "$id" ] || return 1
[ -z "${SUTANDO_TMUX_PANE:-}" ] || [ "$id" = "$SUTANDO_TMUX_PANE" ]On #4418 a blank persisted identity made a comparison vacuously true and wedged a task forever. Here the blank is an observed value — tmux answering a dead pane in a live session with rc 0 and an empty id — and it is rejected by [ -n "$id" ] before any comparison runs. Right guard, right side, and the comment says why rather than restating the code.
I also traced the writer/consumer pair rather than reading one end, which is the mistake that cost me last time:
- Writer:
[ -n "$CORE_PANE" ] && NOTIFIER_ENV_ARGS+=(-e "SUTANDO_TMUX_PANE=$CORE_PANE")— an unresolved pane means the var is simply absent, never blank. - Consumer:
TARGET="${SUTANDO_TMUX_PANE:-$SESSION:$CORE_WINDOW}"— absence degrades to a coarser target (session:window), not a wrong one, and the final clause correctly stops requiring exactness when nothing was declared.
That pair is what makes the last line safe rather than the -z looking like a hole.
Reusing _probe_task_notifier for both the Claude and Codex probes is also the right call — one supervision contract, two adapters, rather than a second copy that drifts.
Why I am not approving
src/agent/claude/cli/start-cli.sh is startup and src/agent/claude/cli/task-notifier.sh is the delivery loop. REVIEW.md lesson 15 is squarely about this diff: the failure mode it names is a supervisor that comes up and supervises nothing, which no harness can distinguish from one that works. You have said the launcher-driven post-restart witness is owner-gated on Chi's restart window — that is exactly the "ASK with its cost named" the lesson describes, and naming it is the process working, not friction.
So: I am not asking you for anything you can produce right now. When that window comes, the witness I would want is narrow — the launcher starting the notifier under the supervisor bound to the real core pane, the notifier surviving, and the health probe going red when it is killed. That last half matters most, because "pages when it dies" is the claim a green harness cannot support.
Two smaller notes for the merge decision: CI has six checks still running at this head, and qingyun's only approval is at c523757b3, stale — so this currently has zero at-head approvals against a bar of two, and mine will not be one of them until the witness exists.

REQUEST_CHANGES at 040d03694 — replaces this login's stale APPROVED at c523757b3. One layer on main (merge-base = ac31aaf0c = main tip, 1 commit, 9 files). Suites at HEAD under env -i: start-cli 15/15 (48 s), health 13/13, worker-mode 13/13 (23 s), notifier 42/42 (133 s), inflight 22/22 (104 s) — the description's counts hold. Both findings fixed here are pinned and mutation-detected: reverting target_pane_is_live → FAIL test_a_vanished_pane_ends_the_wait_at_once; restoring notifier_py="${PY:-$(command -v python3)}" → FAIL test_no_runnable_python_starts_the_core_but_no_watcher_and_says_so. Gates clean (review-checks PASS, no host paths, lint-workspace clean).
Blocker 1 — CI is red at this head, and it is this PR's line
src/agent/claude/cli/start-cli.sh:771 prints … no runnable python3 (scripts/python-binary.sh) … to stderr. tests/start-cli-chrome-seed-bundled-python.test.py:88-90 scans start-cli.sh for a bare python3 outside quotes/word chars, and this prose matches it:
AssertionError: bare `python3` still invoked in start-cli.sh: [' echo " ⚠ task notifier not started: no runnable python3 (scripts/python-binary.sh); …" >&2']
Passes on origin/main (Ran 2 … OK), fails at HEAD (FAILED (failures=2)); that single failure is what turns python standalone tests, diff coverage ≥ 95% ("test failed under instrumentation") and the tsc + tests rollup red (18 success / 3 failed, watched to settle). Reword the message (e.g. "no runnable Python interpreter") and rerun that suite.
Blocker 2 — on by default, and it will type worker-held tasks into the core
This PR is the first thing in production that starts task-notifier.sh (four call sites in start-cli.sh: 862, 933, 1037, 1064; only WORKER_INSTANCE opts out; no flag to disable). What it does on every idle with an empty composer: for each tasks/*.txt from task_dispatch.pending_candidates (no ready result, no name under claims_dir), type Sutando task ready: <file>. Read … and press Enter. pending_candidates (src/delivery/task_dispatch.py:106-130) skips watcher-handler claims only — it never consults deliveries/<recipient>/<id>{.txt,.accepted,.claimed}, the pool router's hand-off sentinel. A task the router delegated to a worker stays in tasks/ by design, so on a pooled host it is a pending candidate.
Measured on this host now: 7 of the 8 files in tasks/ are worker-held (Mars ×4, Sudoo ×3, sentinels present, workers alive). After this merges, the first post-launch idle would keystroke all seven into the core — which is exactly the incident of 2026-09-18 12:55Z (core answered five of Mars's tasks in Mars's rooms), now automated on a loop. The skip has to happen at the candidate step: consult the holder predicate the Stop hook already uses (src/check-pending-tasks.sh claimed_by_a_worker; the pool skill's worker_delivery.holder_of on #4413) before a task is typed, and add a test with a sentinel-held task that the notifier must never deliver. Also state the activation semantics in the PR body — held/parked/chat-tracking task files get delivered on first idle — so the maintainer decides it knowingly.
Non-blocking, for the record
start-cli.sh:747-761/:862: when no#{pane_pid}matches,CORE_PANE=""and targeting silently degrades toSUTANDO_TMUX_WINDOW=${CORE_WINDOW:-0}; on a plain rerun that is window0even if the core healed to:1. One stderr line whenCORE_PANEis empty makes it loud.- A missing supervisor script:
tmux new-session … bash "$NOTIFIER_SUPERVISOR"returns 0, bash exits 127, the session vanishes — silent at launch, visible only via the health probe. Andensure_task_notifierends intmux new-sessionunderset -e, so a tmux failure there aborts the launcher non-zero aftersuccess: core livewas already logged. src/health-check.py:9999: the probe certifies the supervisor's pane + env, never the child — a notifier crash-looping on exit 127 everyRESTART_DELAY=1sreadsok(yixuan's "supervises nothing" green; pre-existing contract, extended here).- Process: the "kewei round-4 findings" are not on GitHub — no review, comment or inline note by keweichen exists on #4410; only the author's paraphrase (issue comment 5736948524). I verified against that paraphrase. The launcher-driven post-restart witness remains owner-gated, as the description says.
- Pre-existing, not this PR:
health-check-codex-task-notifier.test.pyhas 1 failure on both main and HEAD (test_bare_watcher_can_be_green_while_managed_notifier_is_missing, host-state dependent).

My ask was the wrong shape, and that is on me. I asked for ok→red, which assumes a green baseline that cannot exist before the change is deployed. The witness that actually works is three readings: That is strictly stronger than what I asked for: it shows the PR creates the supervised session and that the probe notices its death, in one sequence. It is also fully observable from the warn baseline, so nothing is gated on fixing an unrelated thing. Round 6 — Blocker 1 (CI red on the prose line) was already fixed at 2e90a34; the suite passes at every head since: Blocker 2 (worker-held tasks typed into the core): the skip now happens at the candidate step, in the shared owner rather than a notifier-local check. Tests, at Oracle: with the Activation semantics are now a section in the PR body, as asked. …ding candidate `pending_candidates` skipped watcher-handler claims only and never consulted the pool router's hand-off sentinels (`deliveries/<worker>/<id>.txt|.accepted| .claimed`), so on a pooled host the first idle after this launcher lands would type every worker-held task into the core. The predicate becomes `task_dispatch.worker_holds`, the shared owner: `pending_candidates` / `next_pending_task` take `deliveries_dir`, the CLI takes `--deliveries-dir` (either order with `--claims-dir`, each at most once) and a `worker-holds` command, the Claude notifier passes its deliveries dir at both pick sites, and the Stop hook's bash copy of the same predicate now delegates to the owner. Tests: contract suite pins every sentinel suffix, the absent-option control, the CLI option orders and usage errors (72/72); the notifier-level test drives a live pick with a held task and an unrelated wake and asserts the held task is never typed (inflight 23/23). Checked the new Verified by exercising it rather than reading it: The three-suffix set matters and is right: the router writes The note — an error state collapsing into an actionable value. Fail-open is the right direction here and I want to be clear I am not asking you to flip it — a duplicate delivery is recoverable, whereas returning True on an unreadable dir would skip every task forever, which is the wedge class we already spent two rounds on. The narrow point is that the common case ( My gate is unchanged and this does not move it. I have not re-run the full suites at this head; your contract 72/72 and inflight 23/23 are consistent with the +7/+1 you describe, and the oracle-fails-with-the-skip-disabled control is the right shape for pinning the new branch. |

Blocking at d1c6135cc: src/delivery/task_dispatch.py:120-124 turns every OSError while listing deliveries/ into False ("not held"). A missing deliveries root is the valid no-pool case, but an existing unreadable or transiently unavailable root is "cannot decide". Collapsing both cases authorizes the owner notifier to type a worker-held task into the core precisely when the ownership state cannot be read, recreating the duplicate-execution failure this change is meant to prevent. The worker-holds CLI also maps that uncertainty to exit 1, so check-pending-tasks.sh and notifier callers cannot fail closed. Please distinguish absent from unreadable state, propagate cannot-decide (for example rc 2), make queue selection hold rather than yield on that outcome, and add an unreadable-root regression.
The normal-path implementation and sentinel coverage are good: the focused task-dispatch contract passes 72/72 locally. The notifier integration suite reached all non-process-inspection cases; four watcher cases could not run in the sandbox because ps is denied. Hosted checks are still running.

Both blockers from my 040d03694 review are CLOSED on the Claude path — verified, not taken from the description. One blocker remains, and it is the same defect one file away.
Closed: CI red from this PR's own prose
start-cli.sh:771 now reads "no runnable Python interpreter". tests/start-cli-chrome-seed-bundled-python.test.py → Ran 2 … OK at HEAD; restoring the old wording → FAILED (failures=2) with the scanner naming that exact line, so the cause is confirmed and satisfied rather than assumed. CI at d1c6135cc: 21/21 pass, python standalone tests green.
Closed: the Claude notifier no longer types worker-held tasks
src/delivery/task_dispatch.py:153 — if deliveries_dir and worker_holds(deliveries_dir, name): continue, on worker_holds() at :109 — and the Claude notifier passes --deliveries-dir at both candidate call sites (task-notifier.sh:126 and :431). Behavioral proof on a temp workspace (tasks/task-held.txt + deliveries/w1/task-held.txt, tasks/task-free.txt):
with --deliveries-dir: task-free.txt
without (control): task-held.txt task-free.txt
worker-holds: held rc=0, free rc=1
Pinned twice — test_a_worker_held_task_is_held_back_under_every_sentinel_suffix (task-dispatch-contract, 72 OK) and the end-to-end test_worker_held_task_is_never_typed_into_the_core (claude-task-notifier-inflight, 23 OK, real notifier + fake tmux). Mutation control, removing only those two lines: 9 subtest failures in the contract suite and the live test red. Eleven touched suites all green; review-checks PASS; no host paths.
Also good, and beyond what I asked: src/check-pending-tasks.sh:claimed_by_a_worker now delegates to task_dispatch.py worker-holds instead of keeping its own shell glob — one owner for the sentinel contract.
Blocker — the copy left behind: src/agent/codex/cli/task-notifier.sh:173
The sibling notifier calls the same helper and was not given the new argument:
# codex/cli/task-notifier.sh:173-174
"$NOTIFIER_PY" "$DISPATCH_PY" pending-candidates "$TASKS_DIR" "$RESULTS_DIR" \
--claims-dir "$TASK_HANDLER_CLAIMS_DIR" # ← no --deliveries-dir
# claude/cli/task-notifier.sh:126 (for contrast)
--claims-dir "$CLAIMS_DIR" --deliveries-dir "$DELIVERIES_DIR"Driving that exact argv against the same held/free workspace still returns task-held.txt, and the result goes to tmux send-keys -t "$SESSION:0" -l -- "$prompt" at :244. ensure_task_notifier is called unconditionally from three sites in src/agent/codex/cli/start-cli.sh (355, 460, 477) with no WORKER_INSTANCE opt-out — strictly less guarded than the Claude side. Codex is a first-class core runtime on the same workspace (src/agent/start-cli.sh:53), so a Codex core on a pooled host reproduces the 2026-09-18 12:55Z incident (core answered five tasks held by worker Mars) on a loop.
CLAUDE.md is explicit that this is in-scope, not a follow-up: "do not add a copy, and do not leave one behind… when one defect exists in several places because the policy is duplicated, the duplication is the defect." The shared owner now exists — this is one argument plus a case in the codex notifier suite.
Nit (non-blocking): _WORKER_HOLD_SUFFIXES in task_dispatch.py is a second spelling of pool_delivery.py's PENDING_SUFFIX/ACCEPTED_SUFFIX/LEGACY_ACCEPTED_SUFFIX, with nothing pinning that the two agree. Core cannot import an optional skill, so the copy is defensible — but a fourth suffix added pool-side would silently resume re-delivery. A test asserting the sets match closes it. (#4430 takes the same shape from the other direction: the Stop hook delegating to the skill's CLI.)
Also still owed, unchanged: the launcher-driven post-restart witness is owner-gated, and the PR body still does not state the activation semantics — that after this merges, every parked/held task file in tasks/ is a delivery candidate on the first idle. Say it plainly so the maintainer decides it knowingly.

… never "not held" `worker_holds` collapsed every OSError while listing `deliveries/` into False, so the notifier would type a worker-held task into the core exactly when the ownership state could not be read. Absent root (no pool) stays False; a root that exists but cannot be listed or stat'ed raises WorkerHoldUnreadable. `pending_candidates` holds such a task instead of yielding it (reason on stderr), the `worker-holds` command exits 2, and the Stop hook treats 0 and 2 alike as held so it cannot report the task to the core. Blocking: the unreadable-state fix still fails open when the configured deliveries root exists but is not a directory. In src/delivery/task_dispatch.py:128-129, NotADirectoryError is grouped with FileNotFoundError and returned as "not held." A replaced or misconfigured deliveries/ path therefore makes both the notifier and Stop hook eligible to redeliver tasks even though ownership cannot be checked, contradicting the new contract at lines 119-120.
Please reserve False for a genuinely absent root. An existing non-directory root should raise WorkerHoldUnreadable (and be covered at the function, CLI, and caller layers), just like an unreadable directory.
Focused verification: python3 tests/task-dispatch-contract.test.py passes 74/74 on this head.

Round 8 — Correction first. Round 7's note credited the unreadable-root finding to qingyun-001. That was wrong: it was qingyun-wu's review 5253557757 (Qingyun's Personal Codex) at d1c6135. qingyun-001's review 5253604902 at the same head filed a different blocker, the Codex notifier copy left behind, and round 7 did not touch it. Both are addressed here, credited to their authors. qingyun-wu's blocker at 4664f7a (non-directory root read as not held): only Oracle: with qingyun-001's blocker at d1c6135 ( Also green at GraphQL is rate-limited on this login until 01:21Z, so the CI rollup at this head is unread by me; REST check-runs will say when it settles. …d the Codex notifier passes the deliveries dir `worker_holds` still returned False for NotADirectoryError on the root, so a replaced or misconfigured `deliveries/` path read as "not held" and both the notifier and the Stop hook could redeliver a task whose ownership they could not check. Only FileNotFoundError is the absent (no-pool) case now; any other OSError on the root raises WorkerHoldUnreadable, so the CLI exits 2 and every caller holds. The Codex notifier's `next_pending_task` was the copy left behind: it passed `--claims-dir` only, so a Codex core on a pooled host still selected worker-held tasks. It now passes `--deliveries-dir` like the Claude notifier. tests/codex-task-notifier-worker-holds.test.py extracts the shipped function and drives it against a held task; with the argument removed it picks the held task, with it the free one. APPROVE at 197815bd6. Both blockers standing under this login are closed, and I verified each rather than accepting the round-8 note — including the one that is not mine, because from a shared login my approval replaces it as the latest state whether I intend that or not.
My blocker — the Codex notifier copy — closed.
codex/cli/task-notifier.sh:16 DELIVERIES_DIR="$(dirname "$TASKS_DIR")/deliveries"
codex/cli/task-notifier.sh:176 --claims-dir "$TASK_HANDLER_CLAIMS_DIR" --deliveries-dir "$DELIVERIES_DIR"
grep -c 'pending-candidates' -> 1 # one pick site, and it is the fixed one
tests/codex-task-notifier-worker-holds.test.py 3/3 OK; tests/task-dispatch-contract.test.py OK. Mutation control — removed only the argument: all three fail, including test_without_the_argument_the_held_task_is_picked, a real negative control rather than a restatement of the positive. Behaviorally, that argv against tasks/{task-held,task-free} + deliveries/w1/task-held.txt now returns task-free.txt alone.
The Personal Codex seat's blocker — non-directory root — closed. With deliveries as a regular file:
pending-candidates -> task_dispatch: holding task-free.txt: cannot list …: [Errno 20] Not a directory
worker-holds -> rc 2 (cannot decide)
So an existing-but-not-a-directory root now holds and says why, instead of reading as "not held".
CI at this exact head: 21 pass / 0 fail, watched to settle rather than sampled mid-flight (diff coverage, PowerShell + Windows and python standalone tests were the last three in flight when I wrote the Triage note saying I would wait).
Two notes carried forward, neither gating and both already on the record: _WORKER_HOLD_SUFFIXES is still a second spelling of the pool skill's suffix constants with nothing pinning that they agree (a fourth stage added pool-side would silently resume re-delivery for core readers); and the launcher-driven post-restart witness remains owner-gated, so the activation semantics — every parked task file in tasks/ becomes a delivery candidate on the first idle after merge — are worth one line in the PR body before the maintainer merges.

Approving at 197815bd6a69. My condition was the witness pair on the PR rather than only in the room, and it is there — before warn, after ok naming sutando-core-watcher and task-notifier.sh, on a launcher-started host with nothing touched between readings. CI 21/21 at this head.
You went against my advice on worker_holds and you were right
I said plainly that fail-open was the right direction and asked you not to flip it, because returning True on an unreadable root would skip every task forever. You changed it anyway, and the implementation is better than the binary I offered:
except FileNotFoundError:
return False # absent root = no pool -> deliver
except OSError as exc:
raise WorkerHoldUnreadable(...) # exists but unlistable -> caller must decideSplitting FileNotFoundError from the rest was the half I suggested. Raising a distinct type instead of returning a bool is the half I did not think of, and it is the part that matters: my objection was really that an error state was collapsing into an actionable value, and an exception cannot be collapsed. The caller logs and holds; the CLI returns 2 = cannot decide, distinct from 1 = not held, with the hazard named in a comment. That is three states where I was arguing about two.
Recording this because I was wrong in a way worth being explicit about: I framed a trade-off as binary and pushed for one side, and the right answer was to refuse the frame.
One residual, non-blocking
The hold is now loud in the log — task_dispatch: holding {name}: {exc} per task per sweep — but nothing escalates it. This PR's own check_claude_task_notifier watches the notifier, not delivery throughput, so a permanently unreadable deliveries root would stop delivery indefinitely with only stderr as evidence. Rare (it needs permission or mount damage under the workspace), and the direction is defensible either way, so I am not holding on it. Worth knowing that "loud" here means a log line nobody watches, not a page.
Scope
I verified the delta since my 040d03694 read — worker_holds and its callers — not the whole diff again. qingyun's approval is genuinely at head (submitted 00:42:13Z against a head committed 00:26:24Z, checked by timestamp rather than sha). gating-approvals read 1 of 2 before this; mine should make it 2 of 2.
The killed → warn leg is still untaken and I am not asking for it here — it belongs with the fix_claude_task_notifier follow-up, where both can be witnessed in one restart window rather than apart.

Each now names what tmux actually buys -- the Restart/Stop Core CLI menu items drive the core's tmux pane. The PR body's "No dangling reference anywhere" was false. Its grep was scoped to src/, tests/ and scripts/ and keyed on symbol names, so it established only "no matches for that symbol set under those three directories" -- a repo-wide claim from a three-directory scan. The body now carries that correction rather than a quieter rewording. Left deliberately: CLAUDE.md:333 and its generated AGENTS.md copy, which are owner-gated and not cleanup; and two test FIXTURES whose sample text mentions the feature, since they exercise a parser rather than assert a claim. Stand: Echo Act IV Pro Co-Authored-By: Claude Opus 5 (1M context) <noreply@anthropic.com> * fix(app): drop the Swift core-runtime resolver, whose only caller was its own test `SutandoConfig.sessionCoreRuntime` existed so the watchdog could pick a pane parser before typing. With checkWatcher gone it has no production caller, and its only remaining caller is tests/session-core-runtime-swift.test.py, whose own header names the watchdog defect this PR removes. A dedicated test is not an independent consumer, so keeping the code because that test exercises it is circular. The resolver, its two helpers and the test leave as one unit. `resolveCoreRuntime` in src/sutando_config.ts is a separate implementation with its own callers and its own suite; it is untouched, and tests/sutando-config.ts still exercises it. Stand: Echo Act IV Pro Co-Authored-By: Claude Opus 5 (1M context) <noreply@anthropic.com> * fix(app): keep checkWatcher's detection, replace its recovery keystroke Redirects #4451 per owner discussion: the PR's diagnosis was right (typing `watcher` into a live pane hand-starts a supervised service from outside its supervisor) but its fix -- deleting recovery outright, with the gap left for an unbuilt follow-up -- throws away a working detection half for no reason. Detection is restored verbatim (`watcherProcessSeen`, `watcherLineMatches`, `matchesWatcherScriptAtBoundary`, the 300s Timer, `sessionCoreRuntime` + `SutandoConfig`'s resolver cluster, all Claude-only exactly as before). Recovery is NEW: `checkWatcher()` now shells out to the launcher dispatcher (`src/agent/start-cli.sh --runtime claude`, not the Claude-specific launcher directly -- matches health-check.py's own --recover-core call site) instead of typing into the pane. For an already-running core this hits start-cli.sh's idempotent "already running" branch, which only re-verifies/repairs ensure_core_monitor + ensure_task_notifier and never touches the pane or composer -- so, unlike the keystroke it replaces, it is safe to call regardless of CLI-busy state. `cliIsWorking()`/`processAgeSeconds()` are therefore genuinely dead under this design and are NOT restored. `--runtime claude` is pinned to the runtime `sessionCoreRuntime()` already confirmed live, not left to re-resolve from config: the dispatcher forces a --restart on a config/session runtime mismatch, and re-resolving at call time would risk that racing an already-fine claude core if config had since drifted to codex. CLAUDE.md:333 (+ generated AGENTS.md mirror) updated, not silently deleted: the line told the agent to interpret a `watcher` keystroke, which recovery no longer sends, so the old instruction is dead either way -- this is an owner-visible content change, called out here rather than folded into cleanup, per CLAUDE.md's own rule on removing/changing its instructions. health-check.py, README.md, docs/runtime-dependencies.md, start-cli.sh comments: reverted to describing watcher-auto-restart, since it exists again (just via a different mechanism) -- a2f9b91's "no dangling reference" fixes for the OLD (delete-everything) design are the wrong claim under this one. Tests: app-checkwatcher-ps-fallback (detection-only) and session-core-runtime-swift restored verbatim -- unaffected by the recovery change. health-sutando-app-warn-names-watchdog and app-watcher-recovery-runtime rewritten: the CLI-busy-scoped assertions (cliIsWorking() gates...) are replaced with their inverse (asserting cliIsWorking/processAgeSeconds/tmuxSendLine are ABSENT, and that recovery names the dispatcher, not a pane keystroke). Trimmed to the one sentence that actually instructs the agent: re-arm the watcher yourself via the Monitor tool if you notice it stopped. AGENTS.md regenerated via scripts/agents-md-sync.sh to match. Co-Authored-By: Claude Sonnet 5 <noreply@anthropic.com> * fix(app): ensure_task_notifier verifies the watcher is actually alive qingyun-wu (#4451 review): checkWatcher()'s recovery call hits ensure_task_notifier()'s idempotency shortcut -- tmux session exists, version fingerprint matches -- and returns without ever confirming watch-tasks-stream.sh is actually running. task-notifier-supervisor.sh restarts a crashed notifier on its own, but only while the supervisor process itself is alive; a hung supervisor or a gap inside its RESTART_DELAY leaves the session looking healthy while no watcher process exists. The app reports "repaired" every 5 minutes with the watcher still genuinely absent. Adds watcher_process_alive(), scoped via the same per-instance sentinel PID file watch-tasks-stream.sh's own PID_FILE already uses (util_paths.py watcher-sentinel), not a bare `pgrep -f watch-tasks-stream.sh` -- that would match every worker's watcher on a shared host, the exact unscoped-substring failure this repo already paid for tonight (an unscoped pkill collaterally killed other sessions' production watchers). The idempotency check now requires the session, the version match, AND a live sentinel PID before skipping; any one missing forces a real restart. New behavioral test (tests/app-watcher-notifier-liveness.test.sh): real tmux, a real sentinel PID file, live-PID/dead-PID/no-sentinel scenarios, and a cross-instance isolation check proving a different instance's live watcher can't mask this one's absence. Existing app-watcher-recovery-runtime and health-sutando-app-warn-names-watchdog suites unaffected (18/18, 13/13). #4307 shipped the notifier as a file nothing invokes. On this host it was started by hand today (
nohup), which means a crash or a core restart loses it silently — the exact shape #4307's own witness flagged as the remaining gap. The supervisor was already runtime-agnostic (SUTANDO_NOTIFIER_SCRIPT), so this wires it rather than copying it; moving it out ofcodex/cli/is a separate rename.Activation semantics (read before merging)

Unsampled interleaving, named so the three tasks are not over-read: in all three the pane was busy and a result written before any idle window opened, so the idle-ready+no-result guard never had to decide. The window it would have to guard is "a turn ended with a task read but no result written";_is_idle_readyonly fires on the between-turns footer, and the stop hook refuses to end a turn with an unanswered task, so the reachable case is a turn ending despite the hook (compaction/crash) — the lapse the standby path exists for.
