fix(codex): keep native main available after keyring migration - #7774
A Codex-managed keyring login must reflect the current account on each request. A cached bearer could survive logout or account replacement, while cold Pool selection and stored-Direct audio could miss a keyring-only login.
Cache only home-scoped expiry metadata; obtain the current credential from Codex before dispatch. Pin home and identity across the native-profile claim and honor cancellation. The Codex answer is authoritative per lookup: a withheld token makes the native main account unavailable for the request that asked, and a later lookup that returns a token makes it usable again (upstream still rejects a revoked token).
Reconcile verified account replacement before Direct policy and Pool selection. Bind quota ownership to the exact immutable RPC observation, retire old writers, and preserve manual pause, quarantine and uncertain prior quota evidence.
Use the owned keyring login for account-card metadata, startup quota priming and idle recovery. Stored-Direct audio prepares credentials asynchronously and preserves the existing retryable response on temporary lookup failure.
Distinguish an ordinary no-login observation from a failed refresh. A lookup promoted to refresh, a rejected bearer or a file-bound refresh keeps the existing failure semantics.
Preserve discovery errors for an exact main selector only while the credential remains unusable. After recovery during an entitlement await, use current credential and model-selection evidence instead of an obsolete authentication failure.
Treat a proven missing CODEX_HOME as no login while preserving failures for unsafe or unverifiable paths; recheck caller cancellation and the selected home before returning absence.
Add the setting and canonical behavior link to the Japanese, Korean, Russian and Simplified Chinese configuration pages.
Verification
Author's validation below was recorded at 4e48217 (historical; the current head adds the maintainer changes described at the end), including the missing-home correction and a normal merge of dev 9e48f78. Local validation used pinned Bun 1.4.4 on Windows.
The functional suite results below were obtained at b4b9135. At 4e48217, the latest commit only corrected one portable error-code assertion; its Windows named case passed with four assertions, and the product files are unchanged. Linux CI for this correction has now passed.

The new cases distinguish a genuinely missing home from a file, dangling home link or dangling ancestor. They preserve the no-credential forced-read contract and reject cancellation or home changes during the asynchronous mode lookup. Restoring the old missing-home behavior produced two semantic failures; removing the post-await guards produced two more. The candidate was restored exactly after each control.
The dev merge preserves both the cold-keyring and credit-uncertainty tests. Structural limits initially caught a documentation line and an oversized source file; comment-only reductions passed the affected guards without rerunning the unchanged functional suites. Source/index hashes, bounded execution, native exit/EOF and owned-process cleanup were checked.
Earlier documentation validation at d40f821 passed the frozen dependency install and actual site build: 593 pages and 84,960 internal links. The current follow-up changes no authored docs-site behavior. Prior hosted Cross-platform and Catalog runs passed at that earlier head; The b4b9135Cross-platform CI failed one newly added assertion: Linux correctly rejected a non-directory home with ENOTDIR before the later profile validator. The test now accepts only ENOTDIR or CODEX_HOME_UNAVAILABLE, still requiring an Error, no RPC and unchanged file bytes. No shared write helper or product behavior was relaxed. The 4e48217 head passed Cross-platform CI and Catalog async contracts. No full local repository pass or native installed-keyring acceptance is claimed.
No installed Codex, real account, certificate or global configuration was changed.
Scope limits
Tests use controlled status seams and temporary homes. Live OS-keyring and installed-Codex-login acceptance remains unverified. The synchronous token getter intentionally remains file-only; optional Claude/Chat helper enrichment is outside this change.
The metadata cache is checked after credential/identity reconciliation: its five-minute TTL is not proof of a current login. The explicitly passive account-list path remains available without discovery.
The change is ready for maintainer review. Explicit maintainer security review remains required before merge.
Pool and Direct requests can use a current native Codex login when no stored credential file is available. Verified account changes are reflected in account selection and quota information; uncertain identity or stale credentials do not update quota data.
Responses requests can retry with a verified replacement login after an authentication failure.
Audio transcription can use the native login. Temporary credential lookup failures return a retryable response.
Documentation
Expanded guidance on native login refresh modes, credential discovery, account changes, quota recovery, and failure handling. Added the refresh-mode setting to server configuration references in additional languages.
Maintainer update (lane L7)
Current-head verification (93af71b75, local Windows, Bun 1.4.4, focused files): native-main-refresh-mode 57 pass / 5 Windows skips, main-quota-provenance 37, main-account-credit-renewal 85, codex-credits-probes 7, audio-transcriptions 28, responses-native-main-refresh 21, desktop-compatibility-native-identity 20, file-size ratchet 9, test-layout 2; typecheck, structure, privacy pass. Hosted CI for this head is recorded in the merge comment.
Independent security review found that the earlier "retain permanent refusal until a verified login" rule could not be enforced consistently across every reader and sender of the main bearer. Per maintainer decision the refusal is now scoped to each lookup (see the summary bullet above) and no process-wide refusal state is kept. Kept from review: on a forced-refresh account replacement, both the Responses and compact 401 retries take the quota writer of the exact refreshed observation. The final head merges current dev; verification for it is in the merge comment.
Preserve retryable discovery errors for explicit main selection without overriding a recovered credential or current model eligibility. Keep promoted and forced refresh failures distinct from an ordinary no-login observation, and link translated settings to the canonical contract.
It looks like this branch is under active development. To avoid overwhelming you with review comments due to an influx of new commits, CodeRabbit has automatically paused this review. You can configure this behavior by changing the reviews.auto_review.auto_pause_after_reviewed_commits setting.
The pull request adds Codex-owned keyring credential discovery when auth.json is absent. It connects owned credentials to main-account selection, quota reads, Direct audio authentication, and Responses retries. It also tracks identity and quota provenance, and updates documentation and integration tests.
Main-account refresh can use Codex auth-status results when auth.json is absent. The code tracks credential freshness, home and identity generation, observed absence, and bounded refusal state. Tests cover lookup outcomes, cancellation, home changes, and refresh behavior.

Auth-context selection, Pool recovery, and quota priming prepare and validate owned credentials. The quota probe can use a current prepared token. Tests cover keyring discovery, quota reads, and stale handoffs.

Account identity observation updates lifecycle state and discards mismatched main quota. Tests cover account changes, quota-writer replacement, and persisted quota identity records.
Direct audio authentication resolves credentials asynchronously and maps qualifying native refresh failures to retryable responses. Responses refreshes use the refreshed credential’s quota writer when available. Keyring-backed main-account requests may still run serially, each starting a Codex app-server process, so concurrent requests can queue and time out under load. The accompanying tests can also leak state between cases. Address or explicitly accept the serialization concern and fix the test isolation before merging.

Serialized keyring lookups hold the exclusive claim for every request.
On the fileless path, each resolveMainAccountToken call takes withNativeMainExclusiveClaim at src/codex/main-account.ts:533. Inside the claim, refreshThroughCodex runs the status read. In codex mode, requestCodexAuthStatus starts a new codex app-server child on every call. Concurrent requests therefore run one at a time, and each waits for a child process start and an RPC round trip.
The same 30-second timer covers both the claim wait and the RPC. refreshTimeout is created before the claim is taken (main-account.ts:528-531). The claim receives { waitMs: 30_000, signal }, and that signal includes refreshTimeout. Waiters that exceed the budget can fail as MainAccountTokenRefreshError("transient") (main-account.ts:542-543).
File-backed installs do not share this cost. When auth.json is present and its token is fresh, resolveMainAccountToken returns the token before any claim or child process runs.
The fix is conditional. A read-only refresh: false lookup can use the shared claim only if the Codex app-server does not write the keyring on that call. The repository does not establish that behavior, so do not change the claim until it is confirmed. If it is confirmed, use withNativeMainSharedClaim for the status read. native-main-claim.ts already exports it with the same (context, operation) shape as the exclusive helper. Take the exclusive claim only for the forced refresh path, and re-read status under that claim before the refresh RPC. Coalescing concurrent in-flight status reads per codexHome is an optional change. It does not remove the exclusive-claim serialization.

Cancel quota persistence before restoring the home.
clearAccountQuota(MAIN_CODEX_ACCOUNT_ID) schedules delayed persistence and returns. The callback resolves the restored OPENCODEX_HOME and can overwrite that home’s quota cache.
Use the no-argument cleanup. It clears the remaining quota state and cancels the pending timer.

Exact head: 93af71b75ca85644c363fc446af44d5ffe8a79c4 (author's work, lane fixes, one dev merge).
Exact-head CI: Cross-platform CI run 40605654094 success (attempt 2, restoring the run cancelled by the backlog clear), Catalog async contracts 40605653916 success, Enforce PR target branch 40606269949 success, PR hygiene 40606269902 success.
Security review (gpt-6.1-sol): rounds 1-5 found successive bypasses of the process-wide "permanent refusal" rule. Per maintainer decision (narrow the model) the Codex keyring answer is authoritative per lookup and no process-wide refusal state is kept; the forced-refresh retry quota-writer transfer from round 1 is kept. Round 6 under that model: PASS, 0 blocking, at this exact head.
