fix(codex): validate macOS app trust at execution boundaries - #7758
Stacked on #7778, the test-only ASL fixture prerequisite. Keep this dependency until the parent is merged or closed; retargeting to dev is a separate stack operation.
Keep passive and deferred Codex discovery free of directory-service and signature subprocesses. Validate a selected macOS ChatGPT app executable at each execution boundary, including explicit paths, discovered candidates and memoized selections.
Share the existing ownership, permissions, symlink, strict-signature and OpenAI Team ID policy across sync and async execution. Verification has bounded output, cancellation and process reaping; success is not a reusable trust grant.
Preserve the selected executable's app-bundle classification, Windows batch invocation options, and cancellation through runtime persistence.
Count metadata reads and shared-probe waits against the fixed 15-second dashboard caller budget. Keep external cancellation distinct from timeout and retain the shared flight's fixed deadline. Before spawn, the slot is held until trust verification settles; afterward, it is held until the exact child closes.
Current review follow-up
Current head: 6819eef. Verified merge tree: d0ebecab760803cff335bcab0a7d0696383ca2eb.
The latest normal merge has parents 10b60461a1a44bf1aae0de49718f1222177ad533 and the updated #7778 parent 758a760fe97e72322e923d12832f02af7d2f5c89. It changes only the ASL fixture's cleanup-hook budget and rationale comment. All 15 child follow-up files, including all nine documentation files, retain exactly the hashes validated at 10b60461. No dev refresh or force push was applied. The parent-specific check passed 27 tests / 153 assertions and official typecheck; the child merge tree was verified conflict-free and matches the published tree.
The preceding 10b60461 implementation merge preserved child head 80d62445 and incorporated parent 13e2c00d. No additional dev refresh was applied. The new child delta is 15 files: three product files, three test files, eight localized prompt guides and their owning contract. The parent's one test blob is reused exactly; the final full child PR contains 42 files relative to that parent.
The follow-up addresses all five inline findings:
An abort during asynchronous bundle verification rethrows the original signal reason.
Both sync and async trust-probe failure messages redact secrets and user paths before applying the 160-character limit.
Windows retains the resolved .cmd/.bat target so the launcher constructs its ComSpec /d /s /c wrapper; macOS executes the canonical binary that was checked.
Prompt documentation states the fixed dashboard budget and the slot's pre-spawn trust-verification lifetime, with all eight locales aligned.
The new fixtures wait for trust-entry before aborting, compare the exact cancellation object, save canonical paths before deleting fixtures, and stop runtime handles before restoring spies and removing trees. The fake app-server child has one idempotent termination path for exit code, both output streams and its exit promise.

Against the exact 80d62445 product preimage, the cancellation-identity case, separate sync/async redaction cases and Windows-wrapper case produced the four expected failures. The preimage source hashes stayed unchanged.
bun run typecheck, bun run structure:check, bun run privacy:scan and git diff --check passed. The size ratchet passed 9 tests / 85 assertions.
The final behavioral runs completed with output EOF, child reaping, no live owned process groups and unchanged source/test fingerprints. An initial process observer mistook a terminated zombie for a live process; the final supervisor reaped it and distinguished live from exited descendants. Typecheck itself exited zero; its additional observer had a sandbox PID-namespace mismatch, which is not represented as process-cleanup evidence.
All nine changed documentation hashes match the previously verified build: 593 pages / 84,977 internal links, eight rendered locale paragraphs, 732 unchanged build inputs. That identical documentation build was not repeated.
The original implementation merge matched its locally tested tree. Both parent integrations preserve all 15 child follow-up file hashes; the latest merge adds only the separately verified parent hook-budget change.
The Windows wrapper assertion uses an injected platform and captured spawn arguments. The macOS alias assertion uses a real local symlink and canonical path on Linux. Neither is a new native Windows cmd.exe or macOS app acceptance run. A complete local repository suite was not run under the concurrent-worktree resource exception.
Hosted checks
Current-head Cross-platform CI and Catalog async contracts completed successfully on 6819eef34b09c241318da4a900483284bf1c031c. All four selected Ubuntu shards and the applicable gates/build checks passed. Latest target and hygiene checks passed, and all five inline threads are resolved. This PR is Ready for review. Native Windows/macOS jobs, desktop shell and widget/bundle jobs were policy-skipped and are not counted as native-platform acceptance.
Exact-head full CI
The subsequent full CI run 40439616126 completed successfully at 2026-10-27 03:16:02 UTC on exact head 6819eef34b09c241318da4a900483284bf1c031c: workflow_dispatch, attempt 1, lane=all. All 34 jobs report that head: 28 successful jobs / 6 conditional skips / 0 failures, cancellations or pending jobs. The aggregate job explicitly records LANE: all.
Executed coverage includes all four Ubuntu shards, nine Windows shards, three macOS shards, macOS control, Ubuntu/Windows keyring, Linux desktop Rust checks, macOS widget and app bundle, common gates, storage policy, API usage and Docker smoke.
The six separate conditional jobs npm-global, remote helper, setup action, docs site build, structure gate and privacy gate were skipped in this full run. They are not counted among the 28 successes. This supplements the successful automatic checks above; it does not claim every possible workflow or installed-user scenario was executed.
The original full attempt completed without a rerun. Explicit maintainer security acceptance remains required before merge.
The preceding 10b60461 head passed Cross-platform CI and Catalog async contracts, including all four selected Ubuntu shards and documentation/build gates. Native macOS/Windows jobs were policy-skipped. The earlier Cross-platform CI and Catalog async contracts at 80d62445 are also historical evidence. None of these automatic runs is new native-platform acceptance for the latest head.
Earlier validation retained
Earlier Windows/Bun 1.4.4 validation on the stack included catalog async lifetime (26), Rust async contracts (16), heal reanchor (21), related heal/integration coverage (115, with two POSIX exclusions), and layout/tooling (29). After the original #7778 prerequisite, the provider modality/preset group passed 55 tests / 333 assertions. Those unchanged suites were not repeated wholesale.
Earlier prompt regressions passed 54 tests with one Windows skip / 156 assertions; keep-native V1/V2 suites passed 16 / 132; async persistence cancellation passed 26 / 88 assertions, with four meaningful pre-fix failures. They distinguish elapsed time from timer firing, caller cancellation from shared-flight termination, and result completion from process cleanup.
On the earlier 89a666ba revision, targeted macOS CI passed all three macOS shards, macOS control, and widget/bundle checks. That result remains scoped to its revision. The earlier intermittent messaging-authentication failure and secondary fixture-ownership conflict are not claimed to have proven root causes.
Review readiness and remaining acceptance
Independent static review, focused regressions and the exact-head full CI above cover these follow-ups. Explicit maintainer security acceptance remains required before merge. Earlier-head results remain historical; the newly executed native CI is scoped to 6819eef3 and does not represent an installed-user acceptance or deployment.
Scope is limited to execution-boundary fixes, their fixtures, documentation and the declared parent prerequisite.
Source identities, regression failures and successful checks are documented.
Owning contract and eight localized guides are aligned.
Current-head automatic hosted CI.
Exact-head lane=all full CI, with conditional skips recorded.
Explicit maintainer security review.

New Features
Codex commands launched from macOS app bundles are checked for trust before execution, including login, feature, catalog, prompt, and remote-workspace flows.
Bug Fixes
Prompt-reading timeouts include metadata lookups and shared checks. Expired callers retain locally available base instructions, and external cancellation remains distinct.
Cancelled runtime resolution no longer writes a result to persistent storage.
Documentation
Updated prompt-reading and runtime guidance, including localized prompt guides, to explain timeout and trust-check behavior.

Walkthrough
The changes add shared synchronous and asynchronous bundle-trust checks, validate Codex executables before use across runtime and CLI paths, and update prompt-probe deadline handling. They also change async runtime persistence cancellation behavior and expand tests and documentation for trust checks, cancellation, and execution lifecycles.
Shared filesystem and signature checks support synchronous and asynchronous drivers. The async driver bounds command output and time, and waits for child exit handling after termination. Tests cover policy parity and command-runner behavior.
The boundary resolves Darwin commands and validates known app-bundle targets before execution. Runtime probes use validated targets and retain bundle classification during selection. Tests cover resolution, trust rejection, canonical paths, and runtime selection.
Login, feature, catalog, auth-status, and remote-workspace paths now validate execution targets. Integration tests cover these callers, and test-layout mappings include the new tests.
Async resolve-and-persist returns null when cancellation occurs and does not persist a result. Tests cover cancellation timing, persistence guards, and exception propagation.
Caller budgets include metadata reads and shared-flight waits. Expiry after metadata prevents a new flight; an individual caller can time out while another caller continues using the shared child. Documentation and tests describe these outcomes.

Exact-head full CI is now complete for 6819eef: run 40439616126, workflow_dispatch, attempt 1 / lane=all, finished 2026-10-27 03:16:02 UTC.
34 jobs: 28 succeeded, 6 conditional skips, zero failures/cancellations/pending. All four Ubuntu, nine Windows and three macOS shards passed, together with macOS control, Ubuntu/Windows keyring, desktop shell and macOS widget/bundle. The aggregate log explicitly confirms LANE: all.
The separate npm-global, remote-helper, setup-action, docs-site, structure and privacy jobs were skipped by their conditions; they are not counted as executed coverage. The PR body now records this result alongside the automatic CI evidence. No rerun is needed. Maintainer security acceptance and installed-user validation remain separate from this CI result.

Exact head b12b4e64ae4063e04f0b56bc0dcf12c91cc0cd76 is the PR rebased onto deve1976daf1 as one commit (same 47-file diff as 64fcfaab6; the only conflict was the layout.json explicitShards list, resolved as a union). PR CI is green on this head: all 30 checks (Cross-platform CI 40622203748, Catalog async contracts 40622203869). Windows and macOS lanes were skipped by CI policy. The earlier exact-head lane=all run on 6819eef34 is historical evidence.
Independent security review of the integrated head (64fcfaab6, same diff) reported two High items. Both are in files this PR does not change, and the reviewer classified both as existing behavior that this PR does not introduce: ocx codex (src/cli/codex-launcher.ts) and the generated app-server launcher script (src/chatgpt/app-server-shim/launcher.ts) are not covered by this PR. Those two launch paths still start their executable without the execution-boundary check this PR adds elsewhere. They are tracked in Apply macOS app-bundle execution validation to ocx codex and the app-server launcher #7860, which stays open as a follow-up.
Local (Windows, Bun 1.4.4) on this head: typecheck exit 0; exec-boundary, trust, native-main refresh, runtime, catalog lifetime, prompt probe, remote workspace, launcher, native-priority, layout, ratchet and structure tests 565 pass / 7 skip / 0 fail; structure:check and privacy:scan exit 0. A scratch merge with current dev4cdcb50d9 is clean and passes the new native typecheck (2,192 files, no errors).
