Move the upstream DELETEs out of the reloadMu section.

The cleanup goroutine calls CleanupRemovedSessionRecords while holding reloadMu. Each matching record can block for up to upstream.sessionDeleteTimeout (5s), and later reloads wait for all of them. Run the DELETEs outside reloadMu, and keep the re-add check race-safe.

Review comment at @pkgs/mcpx/internal/daemon/reload.go around lines 116 - 123:
The cleanup goroutine holds reloadMu while CleanupRemovedSessionRecords performs
potentially slow upstream DELETEs. Update the reloadMu section around the
r.pools check and CleanupRemovedSessionRecords so the DELETEs run after
releasing the lock, while preserving a race-safe check that the pool has not
been re-added before cleanup proceeds.

Keep records for a live pool that still serves a configured alias.

The discard step keys on registry names, but a shared pool keeps the name of the server that created it. When a is disabled and alias b still uses that pool, a's records are removed while the session is live. Re-enabling a before a cold restart then cannot resume that session.

Update the configured-name set built from r.pools to retain the registry name of

remain available for resuming the live session.
