Do not classify pre-existing rows by comparing database and dispatch timestamps.

isPreExistingTaskRow compares the database-assigned created_at with the dispatch requestStartedAt using <. If the database clock runs ahead of the dispatch clock, or both values fall in the same millisecond, a live pre-existing task is classified as new. The attach retry is then skipped, and the response reports the task as queued. Use an authoritative inserted-versus-existing result from the enqueue path instead.

Review comment at @pm-stack/dispatch/src/server.ts at line 285:
Update isPreExistingTaskRow and the enqueue flow to use an authoritative
inserted-versus-existing result rather than comparing created_at with
requestStartedAt. Preserve the attach retry for rows identified as pre-existing
and the queued response for newly inserted rows.

Return the existing task for a caller-supplied dedupe_key, even with issue_number.

With issue_number, a repeated request that sends the same dedupe_key queues a second task under a random :dispatch: key. The non-attach path returns the existing task for a caller-supplied repeat. Apply that rule to the attach path too. Keep the retry only for the derived canonical key.

Update enqueueDispatchTask so the attach path returns the existing task when the
dedupe key was caller-supplied, rather than enqueueing a second task with

P2 Badge Preserve caller-supplied dedupe keys for attach requests

When a request supplies both issue_number and an explicit dedupe_key, repeating that request now creates another task instead of returning the existing one: this flag enables the random-suffix retry even though dedupeKey came from the caller rather than the canonical issue key. Consequently, callers retrying an attach request after a timeout can enqueue duplicate work despite supplying an idempotency key. Restrict the dispatch-scoped retry to derived canonical keys and use the existing-task warning path when the caller supplied the key.

Fixed in 97e60a7 — valid catch. enqueueDispatchTask now takes callerSuppliedKey: when the request supplies an explicit dedupe_key (with or without issue_number), a pre-existing row is returned with the warning naming its actual status and no dispatch-scoped retry is attempted — the caller chose that key, so a repeat is an intentional dedupe. The dispatch-scoped retry now applies only to the canonical attach key. New stubbed-queue test covers the combined issue_number + explicit-key case (9/9 in test/dispatch_preexisting.test.ts).
