fix(pm-dispatch): detect pre-existing enqueue row — attach dispatch must not silently vanish (#2677) - #3143
handleTasks derived a canonical (kind, repo, issue) dedupe key for the issue_number attach flow and used the enqueue result unconditionally. The enqueue RPC dedupes against ALL rows incl. terminal ones and returns the pre-existing row — so a second dispatch to an already-tasked issue reported the OLD task as newly queued (201 + "Status: queued" comment) and the new dispatch's prompt silently vanished.
Fix (pinned shape from triage task 4092)
server.ts: capture request start before enqueue; isPreExistingTaskRow = created_at predates request start OR status is terminal (done/cancelled/failed).
attach flow + pre-existing canonical row → retry once with <canonical>:dispatch:<randomDispatchSuffix()> and report that distinct task, with a warning naming the pre-existing task + its status; if the retry also returns pre-existing, explicit warning ("no new task was queued") + pre_existing: true, task_status in the response.
caller-supplied dedupeKey repeat (non-attach) → existing task returned WITH a warning naming its actual status, never as newly queued.
Attach issue comment states the truth: pre-existing → Status: already existing (status: …) — this dispatch did not create a new task; the queue/queued label is only added when a task was actually queued by this dispatch.
queue.ts: stale L8-11 safety comment corrected to cover the attach flow.
Out of scope per packet: enqueue RPC dedupe semantics (#2323), attach target verification (#3007).
Tests
New test/dispatch_preexisting.test.ts (stubbed queue): terminal canonical-key attach, live canonical-key attach, retry-also-pre-existing, caller-supplied repeat, fresh pass-through, detector units — 8/8 pass on laptop (bun test).

New Features
Task submission now identifies when a request matches an existing task and reports its status, rather than presenting it as newly queued.
When attaching to an existing issue, dispatch retries with a separate key and reports whether the retry created a task or matched another existing one.
Issue comments distinguish newly queued tasks from pre-existing tasks.

Explanation
The title names the main change: enqueue can return a pre-existing task row, and the attach dispatch flow must detect it instead of silently dropping the dispatch. The raw summary confirms the server.ts detection logic, the enqueueDispatchTask helper, and the matching tests, and the objectives describe the same behavior. The title is specific and tied to the changeset. The prefix and the issue number (#2677) are acceptable, though the title is long at 100 characters.
Full details: Linked Issues check
Explanation
Issue #2677 coding requirements are met. server.ts captures request start time and detects pre-existing rows by creation time or terminal status. Attach requests retry with a dispatch-specific key. A second pre-existing result returns pre_existing: true, its status, and an explicit warning. Non-attach repeats return the existing task with a status warning. Attach comments report the actual status, and queue/queued is added only for a newly queued task. queue.ts contains the corrected safety comment. dispatch_preexisting.test.ts covers terminal and live attach rows, retry failure, repeated caller keys, fresh rows, and detection units.

Explanation
The changed files support issue #2677. server.ts implements pre-existing-row handling and response/comment behavior. queue.ts corrects the related safety documentation. The new tests verify the implementation with a stubbed queue. No unrelated implementation or test change is shown. Enqueue RPC semantics and attach target verification remain out of scope.

Preserve caller-supplied dedupe keys for attach requests
When a request supplies both issue_number and an explicit dedupe_key, repeating that request now creates another task instead of returning the existing one: this flag enables the random-suffix retry even though dedupeKey came from the caller rather than the canonical issue key. Consequently, callers retrying an attach request after a timeout can enqueue duplicate work despite supplying an idempotency key. Restrict the dispatch-scoped retry to derived canonical keys and use the existing-task warning path when the caller supplied the key.
The reason will be displayed to describe this comment to others. Learn more.
Fixed in 97e60a7 — valid catch. enqueueDispatchTask now takes callerSuppliedKey: when the request supplies an explicit dedupe_key (with or without issue_number), a pre-existing row is returned with the warning naming its actual status and no dispatch-scoped retry is attempted — the caller chose that key, so a repeat is an intentional dedupe. The dispatch-scoped retry now applies only to the canonical attach key. New stubbed-queue test covers the combined issue_number + explicit-key case (9/9 in test/dispatch_preexisting.test.ts).
Add endpoint-level coverage for a pre-existing task.
The current tests stop at enqueueDispatchTask. Add an authenticated POST /v1/tasks test with queue and GitHub stubs. Make both attach enqueue attempts return pre-existing rows. Assert that the response is 201 with pre_existing: true and task_status, that the issue comment contains already existing and not - Status: queued, and that addLabels is not called. Keep the existing helper assertions.

Do not classify pre-existing rows by comparing database and dispatch timestamps.
isPreExistingTaskRow compares the database-assigned created_at with the dispatch requestStartedAt using <. If the database clock runs ahead of the dispatch clock, or both values fall in the same millisecond, a live pre-existing task is classified as new. The attach retry is then skipped, and the response reports the task as queued. Use an authoritative inserted-versus-existing result from the enqueue path instead.

Return the existing task for a caller-supplied dedupe_key, even with issue_number.
With issue_number, a repeated request that sends the same dedupe_key queues a second task under a random :dispatch: key. The non-attach path returns the existing task for a caller-supplied repeat. Apply that rule to the attach path too. Keep the retry only for the derived canonical key.
