• Issue-boundary redaction: CommitmentLog skips WorkRedact, and an install cannot add its own deny patterns

• Hard-pinned vendor: "openai" calls send session and ISA excerpts to a provider the vendor config does not control (ContextSearch --answer, Chain.ts); Assay's ask() skips the redaction its docs promise

• ISA section reading is implemented five times (IsaLadder, Review, IsaTestStrategy x2, isa-utils) with different heading, fence and end-of-section rules

• Paths.ts: add getCodeRoot(), the root of the system code, separate from the data root

• One shared helper for the child-process environment of headless claude spawns (the hand-copied scrubs have drifted)

• Usage tab: cost-trend bars render at 0px (still in 8.761.0, see #1989); model-mix rows wrap the model id and the figures into each other

• Finances: an effective tax rate of 0% in TAXES.md is read as "not found" and replaced by a 25% estimate

• Conduit claudeSession adapter uses the event timestamp as its read cursor, but work-events.jsonl is ordered by file position

• ConfigEvalOnChange single-flight lock: stale takeover (rmSync then wx-create) lets a third run delete a live lock, and an empty lock file is read as stale

danielmiessler/LifeOS#2521 · Filter by author drpaulobernardy opened on Oct 31, 2026

• DeployRegistrationGate treats any Bash call whose command mentions "deploy" as a deploy, so reading a file that contains deploy-shaped output registers a deploy

• With [voice] enabled = false, Pulse does not mount /notify, so every response logs a failed voice event and the silent banner callers (CommitmentSweep, PegWatch) lose their alerts

• Update never refreshes the shared package.json, and OverlaySystem does not check that it is the payload's own copy

• SystemdUser calendar schedule cannot express a weekday (weekly jobs have no systemd translation)

danielmiessler/LifeOS#2517 · Filter by author drpaulobernardy opened on Oct 31, 2026

• InstallSettings merge mode adds no permissions.deny rules to a settings.json that already has permissions, and reports nothing

danielmiessler/LifeOS#2516 · Filter by author drpaulobernardy opened on Oct 31, 2026

• session-identity.ts: without the lock it writes anyway and can drop another session's name, and a lost session-names.json is not recovered from its .bak

danielmiessler/LifeOS#2515 · Filter by author drpaulobernardy opened on Oct 31, 2026

• WorkCompletionLearning: the ISC.json branch fills the criteria text but never closedClaims, so it always logs "No criteria closed" and writes nothing

danielmiessler/LifeOS#2514 · Filter by author drpaulobernardy opened on Oct 31, 2026

• foldToSnapshot overwrites work.json after an unparseable snapshot or unreadable event log, with nothing recorded

danielmiessler/LifeOS#2513 · Filter by author drpaulobernardy opened on Oct 31, 2026

• getIdentity() swallows every LIFEOS_CONFIG.toml error and can name the DA "LifeOS" without ever reading DA_IDENTITY.md

• The nightly deriver counts signals but never says how many lines it could not parse

danielmiessler/LifeOS#2511 · Filter by author drpaulobernardy opened on Oct 31, 2026

• GetCounts reports 0 hooks / 0 skills when settings.json or skills/ cannot be read, indistinguishable from a real zero

danielmiessler/LifeOS#2510 · Filter by author drpaulobernardy opened on Oct 31, 2026

• Pulse syslog write/rotation failures are miscounted or uncounted, and an unreadable Conduit state.json is treated as empty and then overwritten

danielmiessler/LifeOS#2509 · Filter by author drpaulobernardy opened on Oct 31, 2026

• Usage page cannot say why a part is empty: poll and read failures look like "no data yet" (or "Disarmed")

danielmiessler/LifeOS#2508 · Filter by author drpaulobernardy opened on Oct 31, 2026

• IntegrityCheck skips unreadable files and failed git log without a scan error, and three tools write an "Integrity Check" section that checked nothing

• A hung or failed memory-health check, and a failed architecture-summary rebuild, leave no trace the session can see
