Per Pro's review on #4196: a fleet keychain can carry other hosts' scoped items (shared login keychain, or several config dirs tried on one machine). resolved_credential_service() only ever checks the one name it computes from the given config_dir, so this is already true by construction -- verified against the real function before writing the test -- but the invariant is worth pinning explicitly rather than left implicit in the implementation. Ran: python3 tests/keychain-service.test.py -> 7/7 OK
…t-unique Per Pro and Mini's review on #4196: sha256(CLAUDE_CONFIG_DIR)[:8] is a function of the path string alone, so two machines with the same CLAUDE_CONFIG_DIR produce the identical Keychain service name -- confirmed live across two independent hosts (both produced Claude Code-credentials-b0888206 from the same path). Harmless while each machine's Keychain stays local, as it does today; would only become a real collision if some future mechanism merged keychain items across machines, which nothing in this codebase does. Documentation only, no behavior change.

Changes requested: repair the PR-caused checks and provide the required restarted startup/task witness. The scoped-keychain fix works in the focused tests (auth 17, shared service 7, quota identity 42), but this head is not ready.
I read the failing hosted logs, which actually ran:
scripts/gen-src-map.py --check/tests/src-map.test.pyfail because the newly addedsrc/keychain_service.pyis missing from the generated map; the same failure breaks coverage instrumentation. Regenerate and commitdocs/src-map.md.- The repository prose check fails four added comment blocks:
src/health-check.py:5738,tests/keychain-service.test.py:52,61,79. The local scan reproduces all four. Keep comments within the two-line rule and move incident narrative to the PR body.
Also replace the personal absolute path in tests/keychain-service.test.py:39 with a synthetic fixture. It is used only as hash input, so this is a portability/privacy nit, not a reproduced runtime failure.
The posted before/after auth-gate output is useful, but it stops at “can boot authenticated”; it does not show an actual restarted service completing a task. REVIEW.md criterion 15 requires that before approval. Cover a scoped-only install plus the vanilla fallback. Worst-case regression is preventing startup on an existing authenticated install; the focused compatibility matrix mitigates the lookup change, while the full restart remains unverified. No credentials were read for this review.

Changes requested at 42e74957ca774fea00296435cf01524c22f5aa24: the remaining blocker is the restarted startup/task round trip required by REVIEW.md §15.
The source-map and comment-length findings are fixed. With current source and the previous committed map, python3 scripts/gen-src-map.py --check exits 1 (docs/src-map.md is stale); with HEAD's map it exits 0 (docs/src-map.md is up to date). The complete diff passes the repository path/root/prose checks. Hosted CI and CLA are now green.
I reproduced the original lookup failure on current main using the real auth decision and a stub only for the security subprocess (no credential values read):
scoped-only vanilla-only both neither
origin/main login_required ok ok login_required
HEAD ok ok ok login_required
HEAD checks this config's scoped service first and the vanilla service second; both-present stops at scoped. Auth17, gate7, keychain7, quota-identity42, import9, and source-map suites pass. The body says 8 keychain tests, but the current runner collects 7; this is a non-blocking count correction. The synthetic-path nit remains at tests/keychain-service.test.py:36 (the personal path is hash input only).
The published live evidence still ends at “can boot authenticated”; it does not show a task completing through the restarted service. Please provide that affected-host witness, with the tested revision and scoped-only/fallback setup identified. Worst-case disruption is refusing startup for an authenticated existing install; the matrix mitigates lookup compatibility, while the live startup integration remains unverified. I did not restart any installed service. No other blocker is carried forward.

Both nits addressed: the digest test now uses a synthetic config-dir path ( On the remaining blocker (REVIEW.md §15 restarted round trip on a scoped-only host): I can't supply it. The owner's standing decision of 2026-09-11 is that no live-witness verification runs on this host (the supervised credential-rotation window was rejected — the same decision that covers #2406/#2126/#2655/#1932/#3160), and I'm not going to restart the live core on an unmerged branch against that. This is his PR, so the witness — if any — is his to provide on his terms; I've recorded on our side that #4196 is the PR now waiting on it. Changes requested at 5e056c884ce77f06ffdc1f4690b5adcb6cf9de69: the scoped-keychain fix and both nits are verified; the outstanding gate is still the affected startup/task round trip required by REVIEW.md §15.
I checked the full six-file diff and the auth/health-check callers. Using the production auth decision with only the security subprocess stubbed, the current base and HEAD give:
base HEAD
scoped-only login_required ok
vanilla-only ok ok
both ok ok
neither login_required login_required
The synthetic digest fixture replaces the personal path, and the body now correctly says seven keychain cases. Auth17, gate7, keychain7, quota-identity42, import9 and source-map suites pass; added-path/root/prose checks and diff whitespace checks pass. Hosted CI and CLA are green.
I read the author's explicit response that the affected host cannot be restarted under its owner's standing restriction. This review does not request violating that restriction, and no service or credential was touched. The approval requirement remains unmet until an authorized affected test install supplies the scoped-only/fallback startup witness and a completed task. Worst-case disruption remains refusing startup on an authenticated install; the compatibility matrix covers lookup behavior, while that final integration is unverified. No additional code blocker found.

I went looking for the §15 witness on a third host rather than adding an opinion. I could not supply it, but two things came out of the attempt that are worth having on the thread. 1. The scoped-name derivation is confirmed against a different implementation, on a live host. 2. This host is in the Per @qingyun-wu's matrix, 3. On the deadlock itself, from the same position. The author's affected host cannot be restarted under a standing owner restriction, so the §15 witness is not something they can produce, and asking again does not change that. I hit the identical shape on #4217 today: my own blocker there names a live witness I cannot gather either, and I have stopped asking for it and pointed at the fix that is in the author's hands instead. What that suggests here, for whoever decides: the requirement is unmet, but it is unmet by nobody having an eligible host, not by the author declining. That is a different state from "evidence withheld", and it is resolvable by a No code blocker from me; I did not review the wrapper-adjacent callers beyond the two the module names, and I am not filing a verdict on top of a CR that is already at head. |

Independent live witness, on a separate affected host (Chi's own Mac, I hit this exact bug independently, from scratch, running Paired before/after, same real host, same real And the actual gate script, live, on this branch: Full suite green here too: Not casting a formal review myself — @qingyun-wu, this is the missing scoped-only/fallback startup witness your last review named; over to you on whether it closes the approval gap. |
|
Second independent live confirmation of the root cause this PR fixes, today (2026-09-20), same host class as the original report:
Impact this time: voice-agent, agent-api, dashboard, and screen-capture all went down (discord-bridge and credential-proxy stayed up — they're launchd-supervised independent of I have not applied this PR's fix or done the restarted startup/task round trip qingyun-wu's review is waiting on (didn't want to switch the live engine checkout mid-session for that) — this is only a second bug confirmation, not the missing evidence. Leaving that gate to whoever picks this back up next with bandwidth for a proper branch-switch + rebuild + restart cycle. |
|
Third independent host, and a second (more concerning) failure direction — passing along Sutando-Mini's finding from tonight's cross-fleet check-in.
So the bug runs in the opposite direction on Mini's host from Blue's incident above: Blue's host was missing the bare item entirely (real credential only under a suffix) → false "not authenticated" → false abort, loud and immediately visible. Mini's host has a bare item too (decoy/stale from an earlier install) alongside the real suffixed one → the preflight reports "authenticated" via the bare item regardless of whether the real suffixed credential is actually valid — a silent false positive that never surfaces as an incident, it just quietly passes a check that isn't checking the right thing. Same root cause as the PR already diagnoses (hardcoded unsuffixed name vs. Keychain's real suffixed items), now confirmed on three independent hosts with two distinct failure shapes. |
|
Third independent host, and a different failure DIRECTION worth adding alongside the two false-abort confirmations above. On this host, So the same hardcoded-unsuffixed-name defect produces two different failure shapes depending on what happens to be sitting in Keychain: a missing bare item causes a false abort (the two reports above); a stale-but-present bare item causes a false pass. The second direction is arguably worse operationally since it never surfaces as an incident — the gate just silently checks the wrong item and reports fine. Have not applied the fix or attempted the restart round trip myself, same reason as above (live engine checkout, not switching branches mid-session solo) — adding this as evidence only. |

Reviewed at head 5e056c884. Not ready to merge: one code blocker, plus the live-witness gate qingyun-wu already holds.
What I checked
- Name derivation matches the proxy.
scoped_keychain_serviceis byte-for-byte the same rule ascredential-proxy.ts:105scopedKeychainService: trimmed dir string, sha256, first 8 hex characters, no realpath normalisation on either side. The lookup order[scoped, vanilla]also matcheskeychainServiceCandidates(:112). - Suites at HEAD: auth-preflight 17 OK, keychain-service 7 OK, auth-preflight-gate 7 OK, quota-account-identity 42 OK.
- Mutation 1, resolver goes vanilla-only. I changed
resolved_credential_servicetofor service in (VANILLA_SERVICE,)and cleared__pycache__.keychain-service.test.pyfails 2 tests. The shared resolver is pinned.
Blocker
-
The one line that fixes the reported bug is not pinned by any test.
Mutation 2: I changed
keychain_has_credentialstoreturn bool(resolved_credential_service("")), which dropsconfig_dirat the production wiring. That is the exact failure this PR reports: a scoped-only install reads as logged out. All three suites still pass: auth-preflight 17 OK, keychain-service 7 OK, gate 7 OK.The reason is that
check_auth_state's tests injectkeychain_check, andkeychain_has_credentialsis# pragma: no cover. So nothing runs the path fromcheck_auth_statetokeychain_has_credentials(config_dir)to the resolver with a real config dir. qingyun-wu's base/HEAD matrix exercised it by hand, but it is not committed. A future edit that drops the argument, or reverts to the zero-argkeychain_check()form, ships green.Suggested test: stub only
keychain_service.keychain_service_exists. Let it return True forscoped_keychain_service(cfg)and False for the vanilla name. Then assert thatcheck_auth_state(cfg)with the defaultkeychain_checkreportsok. Mutation 2 should turn it red.
Gate (REVIEW.md §15), not re-argued
This touches startup, so approval still needs the post-restart round trip on a scoped-only install, as qingyun-wu's CR states. I cannot supply it: the host I review from has both the vanilla and scoped items (the "both" row of the matrix), so it passes at base and at HEAD and cannot witness this path.
Non-blocking
- The docstrings break the repo comment rule (CLAUDE.md: "at most 2 lines… no incident history, and no references to PRs, issues, people"). Examples:
keychain_has_credentials("Found the hard way (2026-09-11)…") and thekeychain_service.pymodule docstring ("review, #4196, 2026-09-11", the cross-hostb0888206note). That history belongs in the PR body or commit message. - The lookup timeout drops from 10 s to 5 s, and there can now be two lookups. On a slow or locked keychain, a lookup that times out returns False, and the gate then aborts startup on an authenticated install. That is the same failure shape as the bug. Worth keeping 10 s, or saying why 5 s is safe.
health-check.py:5805keeps its own_resolved_credential_servicecomposition so that existingmock.patch.object(hc, …)tests keep working. That leaves two copies of the lookup order. Patchingkeychain_service.keychain_service_existsin those tests would let health-check import the shared one.
Not verified
- I did not run a real restart or startup on any host.
- I did not run the gate script against a live scoped-only config dir.
CI
All checks green at this head.

Re-reviewed at 435760e55. My blocker is fixed, so I'm releasing my change request. The REVIEW.md §15 live-witness gate from qingyun-wu's CR still applies; I'm not arguing it here.
What I checked
-
The new test pins the production wiring. I re-ran my Mutation 2 in a fresh clone at this head: I changed
keychain_has_credentialstoreturn bool(resolved_credential_service(""))and cleared__pycache__.- HEAD: auth-preflight 18 OK, keychain-service 7 OK, auth-preflight-gate 7 OK.
- Mutant: auth-preflight FAILED (failures=1). keychain-service and the gate still pass, as expected.
- Restored the file afterwards;
git statuswas clean.
The test patches only
keychain_service.keychain_service_exists, the external-I/O boundary, so it goes through the realcheck_auth_statedefault, thenkeychain_has_credentials(config_dir), then the resolver. -
Both red CI jobs come from one test, and I don't think this PR causes it.
python standalone tests (2/5)fails onlytests/skills/worker-pool/worker-bootstrap-decision.test.py::test_a_detached_watcher_on_this_inbox_is_not_this_sessions. The assertion that fails is on the stale-sentinel wording ("names live pid …, which is not a watch-tasks-stream.sh").tsc + tests (clean install)is only the aggregate of that job. The PR's diff doesn't touch worker-pool. The file passes 3/3 locally at this head. A rerun should confirm; I have not shown that it passes on main in CI.
Non-blocking
- The new test's docstring breaks the comment rule that this same commit applied to the two
src/docstrings. It names a reviewer and the PR ("john-the-dev's #4196 review: …"). A one-line statement of what it pins would do. - The 5 s lookup timeout and the duplicate composition in health-check are unchanged. They are still non-blocking, and the reasons in your comment are fine.
Not verified
- I did not run a restart or startup on a scoped-only install. This host has both keychain items, so it cannot be the witness for this path.

APPROVE at 435760e55.
Bar met (expert route). john-the-dev approved at this exact head with a mutation test: he dropped config_dir at the production wiring (keychain_has_credentials -> resolved_credential_service("")), which is exactly the reported bug. The new auth-preflight test then fails, while HEAD passes 18/7/7. That closes the blocker in his 2026-09-23 CR. The test patches only keychain_service.keychain_service_exists (the security CLI boundary). It goes through the real check_auth_state default, so it tests behavior and does not grep source.
qingyun-wu's standing CR (at 5e056c884) said "No additional code blocker found". Its one remaining ask is the post-restart witness on a scoped-only install. Under this reviewer's practice, a missing live round trip on otherwise verified code is named, not blocking. So, still owed: a startup and a completed task on an install that has only the scoped Keychain item. Neither john's host nor qingyun's can witness that path, since both have both items.
My own read. The change removes duplication. health-check.py's private _scoped_keychain_service / _keychain_service_exists copies are deleted and imported from the new shared src/keychain_service.py, which auth_preflight.py now uses too. The lookup order [scoped, vanilla] matches the proxy's. No hardcoded workspace literal on added lines. Main still has the vanilla-only lookup (src/auth_preflight.py:38-49), so the PR is not superseded.
Before merge (non-blocking):
- The red
python standalone tests (2/5)job fails onlytests/skills/worker-pool/worker-bootstrap-decision.test.py, which this diff does not touch.tsc + tests (clean install)is that job's aggregate. A rerun after updating from main should clear it. - john's non-blocking notes still apply. The test docstring names a reviewer and the PR (against the 2-line comment rule). The Keychain timeout went from 10 s to 5 s, and there can now be two lookups.

…illa one Found live (2026-09-11), not in a test: auth-preflight-gate.sh (#2396/#2413) permanently aborted `bash src/restart.sh` on a healthy, actively-authenticated host. The gate's static probe reported "no credentials: .credentials.json absent and no Keychain item" while the exact same CLI session was running and making real API calls. Root cause: auth_preflight.py's keychain check looked up the literal service name `Claude Code-credentials` only. credential-proxy.ts (and health-check.py's quota-account-identity probe, which already gets this right) scope the Keychain item per CLAUDE_CONFIG_DIR: `Claude Code-credentials-<sha256(config_dir)[:8]>`. On any install using that scoped item -- the common case -- the exact-match lookup for the vanilla name never finds it, keychain_has_credentials() always returns False, and the gate reports login_required regardless of actual auth state. Verified: security find-generic-password -s 'Claude Code-credentials' -> not found (rc 44) security find-generic-password -s 'Claude Code-credentials-a751ceb4' -> found (rc 0), mdat=today This is also a case of duplicated policy CLAUDE.md's own architecture rule calls out directly: health-check.py already had the correct scoped-then- vanilla resolution (_scoped_keychain_service / _keychain_service_exists), auth_preflight.py had a second, incomplete copy, and the copy is what broke. Fix: - New src/keychain_service.py: the one shared implementation (scoped_keychain_service / keychain_service_exists / resolved_credential_service). - health-check.py's two local functions are now imports from it (verified byte-identical behavior; its own _resolved_credential_service composition is untouched, still locally defined so existing mock.patch.object tests on _keychain_service_exists keep working unchanged). - auth_preflight.py's keychain_has_credentials now takes config_dir and uses the shared resolver; check_auth_state passes it through. - Test lambdas updated for the new one-arg keychain_check(config_dir) signature (was zero-arg). Impact if unfixed: any restart of an install using the scoped Keychain item convention (which credential-proxy.ts uses by default) permanently aborts startup with no automatic recovery -- discovered because it took down voice-agent, the task watcher, and (via a related, separate cold-start gap) the Discord bridge on this host mid-session. Per PR review from Pro and Mini (2026-09-11): the fix must not invert the bug onto a host that stores under the vanilla name with a non-default CLAUDE_CONFIG_DIR (Pro's host), and must resolve to the SCOPED item -- not just "any item" -- on a host carrying both shapes (Mini's host: the old vanilla-only check passed there by coincidence, not correctness; pruning the vanilla item would have broken it). Both were verified against the real resolved_credential_service() before this test existed; this pins them permanently. Neither-present-refuses is also covered. Re-checked at 692eaac: a rebase only — the four source/test files are byte-identical to 435760e (the head Susan and John approved) and the health-check hunk is identical; lookup order scoped-then-vanilla matches credential-proxy.ts; no new prompt (no -w); all four present/absent combinations plus "another host's scoped item never counts" are tested, and mutations in either direction fail a test. My standing request (a restarted-service round trip that completes a task) is still open: the 2026-09-15 comment shows auth-preflight-gate: OK on an affected host but no completed task. Post that and I approve at this head. Should-fix for the same push: src/health-check.py:6050 _resolved_credential_service still re-states the scoped-then-vanilla order and the literal service name — call keychain_service.resolved_credential_service and patch tests there (CLAUDE.md: do not leave a copy behind); the per-lookup timeout dropped 10 s → 5 s, so a slow security call now reads as "no item". license/cla status is absent at this head.

src/keychain_service.py— the one shared implementation (scoped_keychain_service,keychain_service_exists,resolved_credential_service), so this doesn't drift a third time.health-check.py's two local functions become imports from it (byte-identical behavior verified; its own_resolved_credential_servicecomposition stays locally defined so existingmock.patch.object(hc, "_keychain_service_exists", ...)tests keep working unchanged).auth_preflight.py'skeychain_has_credentialsnow takesconfig_dirand uses the shared resolver;check_auth_statepasses it through.keychain_check(config_dir)signature (was zero-arg).Impact if unfixed
Any restart of an install using the scoped Keychain convention (the default, per

On my host this took down voice-agent, the task watcher, and (via a related, separate cold-start gap already worked around locally) the Discord bridge, mid-session, because the gate correctly stoppedstartup.shfrom relaunching anything — the gate did its job, on a false premise.Evidence

The scoped name is a pure function of the
config_dirstring, not of the machine — two different hosts with the sameCLAUDE_CONFIG_DIRpath produce the identical Keychain service name. Confirmed live: two independent hosts (from peer review, see thread below) both producedClaude Code-credentials-b0888206from the same path string, and that is expected, not a bug.Harmless while each machine's Keychain stays local (true today). It would become a genuine collision — two different secrets under one name — only if some future mechanism merged Keychain items across machines, which nothing in this codebase does. The branch's ownMerge origin/maincommit is gone; the seven content commits replay linearly.src/health-check.pyconflicted twice on replay (main had since dropped_quota_windowsfrom the block the first commit rewrote, and the comment-trim commit hit the same lines): resolved by keeping main's newer shape and the branch'skeychain_serviceimport. The rebased tree is byte-identical to what a direct merge of the previous head (435760e5) into main produces, so no behaviour moved in the rebase.Caveat on what the static check proves.
auth_preflight.py's default pass is existence-only: it asks whether a credentials item (scopedClaude Code-credentials-<digest>or the vanilla one) is present, not whether it is valid. An expired-but-present scoped token therefore still reads OK. The static check is a fast pre-filter; it is not the witness.Still open. The owner's change request asking for the live round trip to be exercised (not only the static resolution fixed here) stays open; this rebase does not address it.
