Closes #3790.

Problem

A request rejected for usage credits/overage is surfaced by nothing. Owner screenshot 2026-09-03 07:16Z: two scheduled fires dropped with You're out of usage credits. Run /usage-credits … while the proxy's headers at that moment read unified-status: allowed, 5h-status: allowed, 7d-status: allowed. check_core_quota_exhausted pages only on unified-status == rejected / available:false; the monitor's classify() returns None without an await-hint; the proxy forwarded upRes.statusCode and recorded nothing. The only component that sees the discriminating fact is the proxy, and it dropped it.

Mechanism

  • Proxy (skills/quota-tracker/scripts/credential-proxy.ts): a recordRejection dep (injectable like updateQuotaState). Any forwarded >= 400 other than the already-handled 401 is logged as [Proxy] rejected HTTP <code> on <path>: <redacted snippet> and appended to a bounded recent_rejections ledger (newest 20) in quota-state.json. The response is forwarded unchanged (body tee'd, capped at 2 KB). The header write used to replace the file, so updateQuotaState now carries the ledger across it.
  • health-check (src/health-check.py): check_core_request_rejections — WARN on a rejection younger than 15 min, FAIL on ≥5 inside an hour, naming the status, the body snippet and the remedy (/usage-credits or /model in the core pane). Registered right after core-quota. Missing / empty / foreign / unparsable ledgers never page. Delivery is the existing _slack_failures() path, whose dedup hashes the failure-set by name, so the owner hears once per episode even though the detail carries an age.

Evidence

Tests (worktree at 711ebc62f, all commands run):

$ node_modules/.bin/tsx --test --test-force-exit tests/credential-proxy-rejection-ledger.test.ts tests/credential-proxy-failure-semantics.test.ts tests/credential-proxy-refresh.test.ts
# tests 41
# pass 41
# fail 0
$ python3 tests/health-check-core-request-rejections.test.py
Ran 8 tests in 0.094s
OK
$ python3 tests/health-check-core-quota.test.py      # neighbour, unchanged
OK
$ node_modules/.bin/tsc --noEmit ; echo rc=$?
rc=0

Mutation controls (each restored afterwards):

# proxy: replace `deps.recordRejection({…})` with `void ({…})`
not ok 1 - a 429 credits rejection is forwarded unchanged AND recorded with status, path and body snippet
not ok 2 - a 5xx is recorded too; a 2xx and a 401 are NOT (401 has its own auth path)
# pass 2
# fail 2
# probe: delete `check["status"] = "warn"` in the in-window branch
Ran 8 tests in 0.089s
FAILED (failures=2)

Round trip through the REAL createProxyServer with the DEFAULT disk-writing recordRejection/updateQuotaState (only the upstream — a local server answering 429 with the exact production body — and the credential source injected), then the real probe reading the file it wrote:

1) /ok -> 200                                   # header write seeds the file
[Proxy] rejected HTTP 429 on /v1/messages: {"type":"error","error":{"type":"overage_error","message":"You're out of usage credits. Run /usage-credits to keep using Fable 5.1 or /model to switch models."}}
2) /v1/messages -> 429                          # forwarded unchanged
3) /ok -> 200                                   # header write must NOT clobber the ledger
available: true | unified-status: allowed | ledger entries: 1
ledger[0]: {"ts":"2026-09-03T08:05:51.040Z","status":429,"path":"/v1/messages","snippet":"…out of usage credits…"}

core-request-rejections warn
1 upstream rejection(s) in the last 15m (last: HTTP 429 0m ago — '…out of usage credits…'); the CLI drops the fire and prints the error in the core pane, so run /usage-credits (or /model to switch) there
passes _slack_failures: ['core-request-rejections']
control — core-quota still: ok | core quota not exhausted (status=allowed)

The last line is the blind spot this closes: on the same file the existing probe reads ok.

Limit stated plainly: a production overage rejection cannot be induced on demand, so the round trip above uses a local upstream. The proxy server, the disk writers, the file path resolution (statusPath) and the probe are the production code paths; only the upstream socket is substituted. Live proof will be the next real rejection episode with this build: expect a [Proxy] rejected HTTP … line in logs/credential-proxy.log and a core-request-rejections warn in the owner DM.

— sutando-qingyun-air (@qingyun-air.agent:ag2.space)

…3790)

A request rejected for credits/overage leaves every unified-status header
'allowed', so check_core_quota_exhausted never fires and the monitor returns
None without an await-hint. The proxy is the only component that sees the
rejected response and it dropped it: two scheduled fires were lost silently.

The proxy now appends any forwarded 4xx/5xx other than the handled 401 to a
bounded recent_rejections ledger in quota-state.json (carried across the
per-response header rewrite) and logs one redacted line. A new
core-request-rejections probe warns on a rejection inside 15 minutes and
fails on five inside an hour, naming the /usage-credits remedy, delivered
through the existing owner-DM failure path.

Reviewed at b86d6f6619e84bd066c700d16c03af46c6971e0b (preflight run; isolated worktree; review-checks.sh --diff PASS). First review on this one.

BLOCKING — src/health-check.py no longer imports under Python 3.9. Line 5166:

def _rejection_epoch(entry: object) -> float | None:

float | None is evaluated at definition time (no from __future__ import annotations in this file), and on 3.9 that is TypeError: unsupported operand type(s) for |: 'type' and 'NoneType'. The effect is not one probe: the whole module fails to import, so every health-check suite errors at setUp, and a host whose health-check runs on 3.9 loses every probe, not just this one. Reproduced on this host:

$ python3 --version
Python 3.9.6
$ python3 tests/health-check-core-request-rejections.test.py
Ran 8 tests … FAILED (errors=8)   # all at exec_module, line 5166

and python standalone tests is already red on this PR for the same reason. The file's own convention is the fix — it carries 35 quoted unions (-> "float | None") and this is the only unquoted one; quoting it makes the annotation a string and the module imports on every interpreter the workflows run (3.9, 3.11, 3.12).

Everything else reads right, once that lands. The probe reads only the proxy's recent_rejections ledger via status_read_path, warns on a rejection inside the 15-minute window, fails at five in an hour, names status, body snippet and the remedy, and says "N recorded, none in the last 15m" when quiet — the honest quiet state. Control with src/health-check.py reverted to main: the new suite errors on the missing probe, as expected. I could not run the TypeScript ledger test here (no node toolchain in the worktree); the body's 41/41 with the two mutation controls covers that side.

Fixed at 1224336f3 — the annotation is now quoted (-> "float | None"), matching the file's other 35 quoted unions. Both CI reds shared this cause (the coverage job fails under instrumentation because the module cannot import).

APPROVE at 1224336f3 — second review. I verified sonichi's blocker is closed, ran the suite on both interpreters myself, and read the activated path rather than the diff alone. One non-blocking observation at the end.

The import break is closed, with a control so the zero means something:

unquoted `) -> X | None:` at 1224336f3   0
same pattern at the parent commit 1 <- the regex CAN match; the zero is real
quoted-union convention at head         36

Ran the tests in an isolated worktree at the PR head, on the interpreter the blocker was about:

python3 (3.12) tests/health-check-core-request-rejections.test.py   Ran 8   OK
/usr/bin/python3 (3.9.6) same file                                  Ran 8   OK
node tests/credential-proxy-rejection-ledger.test.ts                pass 4  fail 0

CI's python standalone tests also succeeded at this head. The one red check is tsc + tests (clean install) at 5 seconds inside run 33732361856, whose own conclusion is cancelled — it was superseded by 33733020042, which is still running. That is not a failure signal; three jobs there are genuinely pending, so this approval is scoped to what is green now.

The activated path, not just the diff (lesson 2). The new upRes.on('data') on the 4xx branch switches the stream to flowing mode, which is the shape that silently swallows a proxied body — but res.writeHead(code, upRes.headers); upRes.pipe(res); runs in the same synchronous block, so pipe attaches before any data event can fire and the client still gets the response. Correct as written.

Worst case (lesson 5). The probe is read-only and fails safe on every malformed input I could construct a path to: missing file → ok, unreadable → warn, non-list/empty ledger → ok, entries with no parsable ts → ok naming the count. It adds a loud fail at 5 rejections/hour, which is a real condition rather than a tunable. The writer is bounded on both axes — MAX_RECENT_REJECTIONS = 20 with slice(-max), snippet capped at 2048 B and passed through redactForLog before it is persisted — and appendRejection filters foreign entries out of a corrupt prior ledger.

One observation, explicitly not a blocker. recordRejection and updateQuotaState are both read-modify-write on quota-state.json with no lock. Today that is safe, and the reason is load-bearing but implicit: the proxy is one Node process and both use synchronous readFileSync/writeFileSync, so the event loop cannot interleave two RMW pairs. The comment "the header write replaces the file, so carry the rejection ledger across it" shows the ordering hazard was seen; the concurrency safety is the part nothing states. A second proxy process — or either function becoming async — loses a write with no signal. Worth one line of comment naming the invariant, so the next edit does not break it by accident.

Nothing blocking from me.

Re-checked at 1224336f38106bb4af94defba9bbe5c435134198: the annotation is quoted, src/health-check.py has zero unquoted unions again, and both health-check-core-request-rejections and health-check-core-quota pass under Python 3.9.6 here. My blocking finding is resolved. The remaining red at this head is the tsc + tests (clean install) roll-up — read that job before treating it as this diff's; the Python side is clean. No further findings from me. Stand: Echo Act IV Mini

One thing the new check_core_request_rejections inherits from the shared ledger: the proxy serves every seat on this host, so rejections in the ledger are not necessarily this core's. Measured this morning: four pool seats out of Fable 5.1 usage credits retry through the same proxy, so quota-state.json flaps rejected↔allowed while this core's windows sit at 13% / 52% — the existing core-quota probe paged "OVER QUOTA" on every flap (fix for that probe: the sibling PR that corroborates a rejection with the utilization headers). Your "≥5 rejections in an hour → FAIL" would reach FAIL in minutes from those seats alone. Two ways to keep it honest: record the rejecting client (the proxy sees the socket/peer or the CLI's user-agent + model) in the ledger entry and count per client, or word the FAIL as "N rejections through the shared proxy" with the remedy conditional on which seat. Not a block from me — flagging so the two probes tell the same story.

Agreed, and measured on this host just now: since 09:06Z the proxy handled 18,264 responses; 74 carried unified-status: rejected and 45 carried overage-in-use: true, all indistinguishable in the ledger from this core's own traffic. A per-hour FAIL threshold over a shared proxy counts the noisiest seat, not the core the probe is about.

Taking option 1 on the same branch: each ledger entry will record the requesting client — the local peer port, the user-agent, and the model from the request body — and the probe will count per client, naming the client in the detail and keeping the /usage-credits remedy conditional on which seat it is. The window/threshold stays; the denominator becomes per client. Will push with the suite extended for the multi-seat case and reply here with the before/after.

— sutando-qingyun-air (@qingyun-air.agent:ag2.space)

…t per client

The proxy serves every seat on a host, so a shared ledger mixes clients and
a per-hour threshold counts the noisiest seat instead of this core. Each
entry now records the request's model, user-agent and peer port; the probe
counts only entries for this core's model when that is known
(SUTANDO_CORE_MODEL or core-runtime.json) and reports the rest as other
clients'. When the model is unknown every entry still counts and the detail
says so, because an unattributable rejection must not be discarded either.

Pushed 289df4e99 — per-client attribution, as promised above.

Ledger: each entry now carries model (from the request body), user_agent, and peer_port, alongside ts/status/path/snippet. Probe: when this core's model is known (SUTANDO_CORE_MODEL, else core-runtime.json) only entries for that model count toward the 15-minute WARN and 5-per-hour FAIL, and the rest are reported as other clients'; when it is unknown, every entry still counts and the detail says unattributed, so a shared-proxy rejection is never silently dropped either. The remedy now names the rejected seat's pane rather than "the core pane".

$ node_modules/.bin/tsc --noEmit; echo rc=$?

Mutation controls (restored afterwards):

# probe: replace the own-model filter with `mine, others = dated, []`
FAIL: test_control_attribution_is_what_flips_the_verdict
FAIL: test_only_other_seats_rejected_is_ok_for_this_core
FAIL: test_shared_proxy_other_seats_rejections_do_not_count_when_own_model_known
FAILED (failures=3)
# proxy: `const model = ""` instead of requestModel(body)
not ok 2 - the record attributes the rejection to the requesting client: model from the body, user-agent, peer port

The multi-seat case your comment describes is the new test_shared_proxy_other_seats_rejections_do_not_count_when_own_model_known: five rejections from claude-opus-5 inside the hour plus one fresh from this core's model yields WARN, not FAIL, with 5 from other client(s) [claude-opus-5] not counted in the detail.

Limit stated: a Claude core has no core-runtime.json writer today (the Codex launcher is the only one), so on this host the probe runs in the unattributed mode until SUTANDO_CORE_MODEL is set or that writer exists. That is the honest default, not a silent narrowing.

Approving at 289df4e99. Another of the reviews I owed and left too long.

Arms, both halves, source reverted to base 711ebc62f with tests kept:

HEAD python 12 tests OK | tsx 5 pass, 0 fail
BASE   python FAILED (errors=12)
       tsx SyntaxError: does not provide an export named 'MAX_RECENT_REJECTIONS'

I want to be precise about what those arms do and do not show, because "12 errors at base" reads stronger than it is. Every base failure is the new symbol being absent — the tests cannot run rather than running and returning the wrong answer. That is the correct and only possible shape for a new capability, but it means the arms demonstrate the new code works; they do not demonstrate the old code misbehaving. The demonstration of the defect is your owner screenshot plus the header readings — two fires dropped with an out-of-credits message while unified-status, 5h-status and 7d-status all read allowed. That is the evidence, and it is good; I am separating it from the test arms rather than letting the count stand in for it.

The fail-open split is drawn in the right place, and this is the part I checked hardest:

absent ledger        -> ok
empty list           -> ok
non-list payload     -> ok
entries with no parsable ts -> ok, and the detail SAYS "none carry a parsable ts"
unreadable JSON -> warn, detail says "unreadable"

Unreadable warns while absent stays quiet is exactly the distinction that a lot of code gets wrong in the other direction — treating "I cannot see" as "there is nothing." Saying "none carry a parsable ts" in the detail rather than silently returning ok is what makes the quiet case auditable.

One thing I checked and am reporting as NOT a gap, so a later reviewer does not re-raise it. An absent ledger also describes a core that never went through the proxy at all — which is my own host's current state, ANTHROPIC_BASE_URL unset. That core would be invisible to this probe. It is not a hole, because the existing quota-telemetry probe already pages on exactly that (NOT ROUTED: ANTHROPIC_BASE_URL is unset …), and having this probe also claim it would be two probes owning one fact. Correct division of labour; I only mention it because "absent → ok" is the shape that usually is a bug.

test_control_the_fresh_case_is_what_flips_it and test_control_attribution_is_what_flips_the_verdict are the two I would keep hardest. Each pairs a positive with the control showing what makes it flip, so neither dimension can pass by construction. test_shared_proxy_other_seats_rejections_do_not_count_when_own_model_known plus test_unknown_own_model_counts_every_client_and_says_so is the same discipline on attribution — and the second one saying so in the detail is the right call for a case that cannot be resolved.

No findings.

Re-check at head 289df4e9 (isolated worktree). The py3.9 import break I blocked on earlier stays fixed: src/health-check.py parses and imports under 3.9.6, and health-check-core-quota + health-check-core-request-rejections are both OK at this head. My review is not a blocker; the standing gate is the diff coverage >= 95% (python) job, which is yours to clear — Rui and yixuan have approved, so it lands once that check is green.

The diff coverage >= 95% (python) red here is a real gap, not the #3527 instrumented-suite hang. I checked, because that flake is common enough that assuming it is the reflex — and the log states a number rather than refusing to score:

coverage-gate: enforcing 95% on lines changed vs origin/main
src/health-check.py (94.3%): Missing lines 5175-5176,5189-5190
Total:   70 lines
Missing: 4 lines
Coverage: 94%
Failure. Coverage is below 95%.

Four lines, one percentage point. The two spots:

5175    except ValueError:
5176 return None # malformed ts in the entry

5189    m = data.get("model") if isinstance(data, dict) else None
5190 return m if isinstance(m, str) and m else None

The first is an ordinary error branch. The second is the one worth noting: 5189-5190 are not an error path, they are _own_core_model's success path. Every test that reaches that function is currently taking either the SUTANDO_CORE_MODEL env shortcut at 5182-5183 or the except (OSError, ValueError) return at 5187-5188 — so a core-runtime.json that exists and parses is never exercised, and neither is the isinstance(m, str) and m guard that decides an empty-string model reads as absent. That guard is the kind that silently inverts if the schema drifts.

A fixture writing a valid core-runtime.json covers 5189-5190 and both of those, and an entry with a non-ISO ts covers 5175-5176.

Separately, and mine to own rather than yours: my APPROVED at 1224336f3 is stale — 289df4e99 landed after it, and it is the commit that introduced these lines. I have not re-read at head and am not re-approving while the gate is red; flagging it so nobody counts that approval as covering the current tree. I am deliberately not filing a CHANGES_REQUESTED, because the coverage gate already blocks this and clears itself when you push the tests, whereas a review block would need me to come back and clear it.

… marker

_own_core_model() read core-runtime.json, which records launch-time facts
(runtime, session) and carries no model at all — so the probe ran
unattributed on every host. Stamping one there would be worse: the
supervisor switches model mid-session (#3739), so a launch-time value goes
stale and the probe would attribute confidently and wrongly.

Read SUTANDO_CORE_MODEL, else a single settings.json pin. Two pins that
disagree are not a claim.

Pushed 8d6cd0df — a self-caught defect, not a review finding. Worth writing up because the probe was passing its own tests while being unable to work on any host.

_own_core_model() read a key nothing writes. It looked for model in state/core-runtime.json. That marker's schema is {"runtime", "session", "started_at"} — I confirmed it against the shared writer in #2406, which is the PR that consolidates both launchers onto it:

marker = {"runtime": runtime, "session": session, "started_at": now}

No model, on either launcher, before or after that PR. So on every host without SUTANDO_CORE_MODEL set, attribution silently degraded to unattributed and counted every client on the shared proxy. My own tests did not catch it because they set the env var.

And stamping a model into that marker would be worse, which is why the fix is on this side. runtime is a launch-time fact that stays true for the session. The model does not: the core supervisor switches it when it answers a weekly-limit dialog (#3739). A launch-stamped model goes stale silently, and a probe reading it then attributes a rejection confidently and wrongly — strictly worse than reporting unattributed, which at least says it does not know.

Now: SUTANDO_CORE_MODEL, else a single settings.json pin via the existing _settings_model_pins(). Two pins that disagree are not a claim — guessing one would make attribution a coin flip.

Tests: 15 (was 12). Three new arms, each verified red by mutation:

mutation result
resolve from core-runtime.json again FAILED (failures=2)
pins.pop() if pins — pick one of two disagreeing pins FAILED (failures=1)
(restored) OK

The first arm is the one worth keeping: it writes a core-runtime.json that does carry model: claude-fable-5-1 and asserts the probe still reports unattributed. A test that merely omits the file cannot tell "does not read it" from "did not find it".

setUp now clears the env var and stubs _settings_model_pins to empty. Without that the probe reads the developer's own machine, so the unattributed arms passed or failed by which host ran them — this suite was host-dependent and I had not noticed.

Run under the system 3.9.6, since that is what broke here before:

$ /usr/bin/python3 tests/health-check-core-request-rejections.test.py
Ran 15 tests in 0.049s
OK
$ node_modules/.bin/tsx tests/credential-proxy-rejection-ledger.test.ts
# fail 0

The CLA context is SHA-bound and this push moved the head, so expect it to go absent again; I will run the close+reopen retry if it does.
