Fix modeler read validation

Summary:

• Harden modeler accessor and buffer view reads against malformed documents.
• Validate sparse indices, byte strides, declared buffer lengths, and normalized integer semantics.
• Add regression coverage for malformed reader inputs and binary slice/stride guards.
