Summary

After a ChatGPT account drops to the Free plan, the Codex surface answers a model the plan can't use with a 400 that has no error code:

400 {"detail":"The 'gpt-6-astra' model is not supported when using Codex with a ChatGPT account."}

comboFailureDecision classified this as a terminal invalid_request_error. So a combo whose first target was that model, [openai/gpt-6-astra, anthropic/claude-opus-5-5], returned the 400 to the client instead of trying the next target. The live gateway this came from returned 400 on every turn until patched.

The refusal concerns this account and model, not the request, so it is now a hop. The match is the exact Codex phrasing: src/codex/catalog/native-models.ts already documents it as the refusal for an ungated model the account cannot reach. Any other 400, including other prose about the same model, stays terminal. The failed target cools at target scope (the existing default), so later turns skip it.

Verification

  • bun run typecheck: clean.
  • tests/routing/router-combo-failover-classification.test.ts: 60 pass. The new hop/stop case fails with src/combos/failover.ts reverted.
  • tests/routing plus tests/codex-integration/combos.test.ts, compared against the same tree with the source reverted: no new failure names.
  • file-size-ratchet, structure:check, privacy:scan: pass.
  • Live: on a fork build with this change, the combo answers 200 through the second target, and the log reads [combo] codexfirst: openai/gpt-6-astra failed with 400 after 2261ms: The 'gpt-6-astra' model is not supported when using Codex with a ChatGPT account.

Not run: the full bun run test.

Checklist

  • Scope stays focused and avoids unrelated cleanup.
  • Docs or release notes were updated when needed. No structure doc enumerates the hop rules, and responses-failover.md is at its 600-line budget.
  • Security-sensitive changes were reviewed for secrets, auth, and unsafe defaults.

Review readiness checklist

This PR stays in draft until every box below is ticked. Tick all four boxes once the requirements are met:

  • Required local validation passed; commands, results, and any full-suite exception are documented.

  • I pushed my PR to a recent dev commit (at most 10 behind; a maintainer may still ask for the exact tip before merge).

  • I resolved all correct Codex and CodeRabbit findings.

  • My PR is ready for review.

Summary by CodeRabbit

  • Bug Fixes
    • Requests rejected with the specific unsupported-model refusal from a ChatGPT account can now continue to the next available target. This applies whether the refusal appears as a plain message, in a detail field, or in an error message. Generic invalid-model errors and unrelated messages remain terminal.

Walkthrough

The failover classifier recognizes the exact ChatGPT Codex account/model refusal in an HTTP 400 response and returns "hop" so the next combo target can be tried. Tests cover supported message forms and terminal nonmatches.

src/combos/failover.ts, tests/routing/router-combo-failover-classification.test.ts
The classifier accepts the exact refusal as a bare message, in a detail field, or in error.message, optionally after removing a Provider error 400: prefix. It rejects non-400 statuses, messages over 16,384 characters, and nonmatching fields. Matching responses return "hop"; tests verify that generic invalid-model text, unrelated JSON fields, and refusal text embedded in other prose remain terminal.

Change: Bug fix

Merge Risk: 🔵 Low · up to ce2c1

A conflicting error response can make a combo skip a target unnecessarily. Reject dual-envelope bodies before relying on this failover behavior; the risk is bounded to ambiguous responses.

Security Architecture Review

Security architecture risk: 🔵 Low · up to ce2c1

A rejection tied to one account may temporarily suppress a destination that other accounts could still use. Requests remain limited to destinations already configured, and no expanded access was established.

Retained concerns

  • Low · reliability · inferred: An account-specific refusal now reaches shared target cooldown recording. The cooldown key contains the combo and provider/model, not the refusing account, and the account-scoped exception applies only to identified 429 failures. If another account can still serve that target, subsequent requests sharing the combo can nevertheless skip it until expiry. Existing bounded alternate-account recovery mitigates this case but does not establish that every remaining account is unable to serve before the combo target is cooled.
Security review details

Security Blast Radius

  • inferred — The new cooldown effect is limited to the failed provider/model within a combo in the shared process registry. It can affect later requests using that combo, but inspected source does not establish cross-tenant, cross-process or environment-wide exposure.

Security Findings and Attack Paths

  • inferred — A controlled upstream can emit the matching refusal and influence failover, but already had other hop signals such as 429 and server errors. The inspected change does not establish new arbitrary-destination authority or an authentication bypass; its newly identified effect concerns shared cooldown ownership.

Trust Boundaries and Controls

  • observed — Cancellation, structured origin refusal, cyber-policy refusal and non-replayable failure checks precede the new hop. The caller also forces non-replayable failures to stop. Alternate selection retains payload, reasoning-replay and protocol eligibility checks.

Resilience and Maintainability Implications

  • observed — Existing account recovery preserves fixed-account restrictions, refuses alternate-account replay for uploaded-file conversations and uses dispatch reservations. These controls mitigate unsafe replay, but are separate from the combo cooldown key and do not make the new 400 cooldown account-scoped.

Reject dual-envelope bodies before classifying the refusal.

When a 400 body contains both detail and error, this classifier selects detail first. If detail contains the exact refusal, combo failover can treat the ambiguous body as a refusal and hop to the next target. Reject bodies that contain both envelopes before reading either field.

Superseded by #6527, now merged into dev as ab68539036fa89e0734ea21aa6b525ae4cc3ae36. The carry retains source attribution and implements plan/model refusal fallback with bounded HTTP/SSE evidence, mixed-envelope ambiguity rejection, and existing cancellation/output-commit/send-limit precedence.

Corrected carry head af350a1924ee55ddc60fe9fa04289d1aa494f437 passed 374 focused tests, independent security/code review, and applicable exact-head PR CI run 37146395644. This records integration; final combined regression, full-platform candidate CI and production publication remain pending. Closing this original proposal as superseded, not as independently merged.

Carry lidge-jun#6505 by Vadym O (ce2c1a6). Permit bounded exact ChatGPT plan/model refusals to try the next declared target, while conflicting envelopes stop before code-based hops. Preserve finite classification before display truncation and SSE reconstruction; never carry raw bodies in that metadata.
