fix(live): relay sideband turn metadata and client closes, and stage realtime voice fixtures - #4748
Conversation
…realtime voice fixtures (#4721) Realtime voice v3 is reported as one broken feature but is really three failures: an immediate 502 on call-create, a ten-second start timeout, and a call whose audio works while no transcript and no tool invocation ever appear. Nothing in the suite could tell them apart, because the sideband had no per-stage coverage and its only diagnostic recorded relayed frames -- so a join that never reached the proxy, a join the upstream refused, and a live relay carrying nothing all produced the same empty file. That is why the report could only say "no frame log". Six de-identified Frameless v3 fixtures now drive the real relay one stage at a time: join, session preamble, audio, transcription, delegated tool work, and shutdown. Their event names and payload shapes follow the v3 parser in codex-rs (session.started, input_transcript.added, turn.done, delegation.created, input_audio.append, delegation.context.append); the first draft used public Realtime v2 names, which do not exist on this wire. A failure now names the stage instead of the feature. Two defects the fixtures pin down: - x-codex-turn-metadata is on the header list codex-rs builds for the sideband upgrade and was missing from LIVE_CLIENT_PROTOCOL_HEADERS, so every realtime turn reached the model with metadata the client had attached and this proxy dropped. The Responses passthrough already forwards it and the sideband addresses the same upstream. Relayed only when the caller sent it; the images sidecar still strips it deliberately. - A downstream close was reported to the upstream as a bare 1000 regardless of what the client sent. A Frameless v3 client reads upstream 1000 as the session completing and anything else as a transport loss to reconnect, so the code is load-bearing. clientCloseForUpstream substitutes 1000 only for codes no endpoint may send and truncates the reason at the 123-byte control-frame limit. OCX_LIVE_FRAME_LOG additionally records upstream-open, upstream-failed, relay-attached, and relay-closed. Both record shapes stay content-free: no URL, no call id, no header, no frame bytes. Not fixed, and deliberately not forced: the upstream handshake status. Bun's client WebSocket surfaces only opened or failed, so a refused join can only become 502/504. Upstream preserves that status (openai/codex #39257) and the client prints whatever we return, which is why the report shows a bare 502. Closes #4721

Preserve close code 1002 when relaying client closes
When a downstream client reports the standard protocol-error close code 1002, this predicate rejects it and converts it to 1000, even though the preceding comment explicitly identifies 1002 as a meaningful protocol fault. Bun accepts and transmits 1002, and this file's upstream-to-client validation already treats it as valid, so the conversion makes the upstream interpret a transport failure as graceful session completion and can suppress the Frameless v3 reconnect. Add 1002 to the sendable set and cover it in the shutdown fixture.

… live suite The file-size ratchet failed the previous head: tests/server/server-live.test.ts sits exactly at its committed cap of 2253 lines, and the lifecycle-aware rewrite of its frame-log assertion pushed it to 2262. Raising the baseline is not available -- the ratchet only ever lowers caps. The assertion belongs in the new file anyway, since that is where the new behavior is owned. server-live.test.ts keeps its existing six-line block and only widens the key check in place, so both record shapes stay bounded and the file returns to 2253. The new test earns its place rather than just relocating the old one: it drives a refused join and a completed relay into one log and asserts upstream-failed, upstream-open, relay-attached, and relay-closed are separate records, with the status on the refusal only. That is the distinction the issue needs and the reason the constructor-failure branch of openLiveSidebandUpstream now records a stage too -- it resolves directly instead of through finish(), so it was the one refusal that still left no trace.

🎯 Functional Correctness | 🟠 Major | ⚡ Quick win
Preserve all valid standard close codes.
websocket-handler.ts:321-330 forwards the client close through clientCloseForUpstream, and live-sideband.ts:255-280 passes the returned code to Bun's WebSocket.close. Bun permits 1000–1014 except 1004–1006, plus 3000–4999. The current predicate maps 1002, 1012, 1013, and 1014 to 1000.
This violates the relay contract in structure/runtime.md:376, which requires transparent close propagation. A client protocol error, service restart, retry-later signal, or upstream-gateway failure can therefore appear to Frameless as a normal session completion.

Summary
Realtime voice v3 arrives as one broken feature but is really three failures with different
causes: an immediate 502 on call-create, a ten-second start timeout, and a call whose audio works
while no transcript and no tool invocation ever appear. Nothing in the suite could separate them.
The sideband had no per-stage coverage, and its only diagnostic recorded relayed frames, so a join
that never reached the proxy, a join the upstream refused, and a live relay carrying nothing all
produced the same empty file. That is why the report could only say "no frame log".
Six de-identified fixtures in
tests/fixtures/realtime-voice-sideband/now drive the real relay onestage at a time: join, session preamble, audio, transcription, delegated tool work, and shutdown.
A failure names the stage instead of the feature.
Their event names and payload shapes follow the Frameless v3 parser in codex-rs
(
protocol_frameless_bidi.rs,protocol.rs):session.started,input_transcript.added,output_transcript.added,turn.done,output_audio.delta,delegation.createdinbound, andinput_audio.append,delegation.context.append,session.closeoutbound. v3 has nofunction_callframes at all: local tool work is a delegation, and it travels only on this socket, because the WebRTC
oai-eventsdata channel ignores every inbound message. That is the mechanismbehind audio-without-tools.
Why the fixture rewrite mattered
The first draft of these fixtures used public Realtime v2 event names. Read against the checked-in
v3 parser they are not near-misses, they are absent:
parse_frameless_bidi_eventaccepts exactlysession.started,session.updated,output_audio.delta,input_transcript.added,output_transcript.added,turn.done,delegation.created, anderror, and logs everything elseas unsupported. A suite built on v2 names would have gone green against a relay that could not carry
a single real v3 frame, which is the worst kind of false negative for an issue whose whole
complaint is that frames do not arrive.
The tool-call difference is not a rename. v3 has no
function_callframes and noconversation.item.createresult: the server asks the client to do local work withdelegation.created, whose item must carrytype: "delegation"andtarget: "client", and theclient answers with
delegation.context.appendkeyed bydelegation_item_id. That is a differentlocal tool-invocation model, so a fixture shaped like a v2 function call would exercise a code path
the desktop never takes. The
toolsstage models the real exchange in its real order: the clientspeaks, the server delegates, the client returns the result.
Two defects the fixtures pin down:
x-codex-turn-metadatawas dropped on the sideband. It is on the header list codex-rs buildsfor the upgrade, so every realtime turn reached the model with metadata the client attached and
this proxy silently discarded. The Responses passthrough already forwards it and the sideband
addresses the same upstream, so there is nothing to scope it away from. Relayed only when the
caller sent it; the images sidecar still strips it deliberately.
A client close was reported upstream as a bare 1000. A Frameless v3 client reads upstream 1000
as the session completing and any other code as a transport loss to reconnect, so the code is
load-bearing and rewriting it is not a transparent relay.
clientCloseForUpstreamsubstitutes1000 only for codes no endpoint may send, which
upstream.closethrows on, and truncates thereason at the 123-byte control-frame limit by bytes rather than characters.
A refused join could leave no trace at all.
openLiveSidebandUpstreamresolves theconstructor-failure branch directly instead of through
finish(), so the one case where theupstream socket could not even be created was the one case that recorded nothing. That is the
worst place to be silent, because it is indistinguishable from a join that never reached the
proxy. It now records the same
upstream-failedstage as every other refusal. This surfacedonly while writing the lifecycle test; it is not a fixture change and should be reviewed as a
defect fix.
OCX_LIVE_FRAME_LOGadditionally recordsupstream-open,upstream-failed,relay-attached, andrelay-closed. Both record shapes stay content-free: no URL, no call id, no header, no frame bytes. With them the three reported symptoms are distinguishable from one file: no record at all means the
join never reached the proxy,
upstream-failedcarries the status the client was handed, andrelay-attachedwith no following frame record means the transport is live and the silence isabove it.
Deliberately not fixed. The upstream handshake status cannot be recovered here: Bun's client
WebSocket surfaces only opened or failed, so a refused join can only become 502/504. Upstream does
preserve and display whatever status it receives (openai/codex #39257 turns a failed upgrade into
"unexpected status
: realtime websocket handshake failed"), which is why the report shows abare 502 from us rather than the real upstream code. A synthetic status would be worse than an
honest one. No
Sec-WebSocket-Protocolhandling was added either: codex-rs requests no subprotocolfor this sideband, so there is nothing to preserve.
Closes #4721
Verification
No local verification was run for this lane. Per repository owner instruction, no local test suite,
individual test file, typecheck, front-end build, or dependency install was executed. Evidence is
static review plus hosted CI at the exact head SHA.
Static checks performed:
not from memory; the first draft used public Realtime v2 names and was rewritten after that read.
scripts/test-layout/layout.jsonexplicit map andtests/fixtures/test-layout-expected.jsoncompared programmatically and confirmed identical, with the new entry present in both.
scripts/privacy-scan.tsdetectors read and each fixture checked against them; a newde-identification test in the same file re-asserts them so the fixtures cannot drift.
updated to bound both record shapes rather than weakened, and the call-create header tests do not
pin the length of the relayed-header list.
Hosted CI on this branch head is the gate.
