Modes: default to plan, retire the pulse-designer agent shell #389

Important

Problem — The mode picker shows plan / build / pulse-designer, but pulse-designer is a shell: its definition is a four-line prompt deferring to the AGENTS.md interview section, and its permission grants sit at the config root applying to every agent identically. The default agent is unset, so sessions boot into build (full write permissions) by silence. Per #368's roles-not-modes direction, the chat transport keeps plan/build as permission envelopes only — the interview lives in the compiled score content, not in a mode.

Acceptance Criteria

  • Sessions boot into plan by default (the generated config sets the default agent), including fresh installs
  • The pulse-designer agent no longer appears in the mode picker; the interview content (the compiled score section in AGENTS.md) is unchanged and visible to every agent
  • The onset router and the amicode_* recording tools function in plan mode; the first solve launch or file write prompts an explicit exit to build
  • No app-shell or server code hardcodes pulse-designer as the selected agent (audit the first-run / session-open path)

Source

Relates to #368 (plan/build survive as permission envelopes). Design conversation 2026-08-16.
