test(core): supervisor-reload 'organization integration changes' test fails on main #26

Seen 2026-10-17 while working #18 (PR #25), on Bun 1.4.2 in this sandbox.

packages/core/test/plugin/supervisor-reload.test.ts — "applies organization integration changes to running external plugins" fails on unmodified main (verified by stashing the PR's test additions and running the single test in isolation): the retry loop waiting for policy-greet exhausts with error: pending (~6.5s isolated, ~3.8s in the full file run).

All other tests in the file pass, including the reload/recovery tests around it. Unclear whether this is an environment-specific flake (timing/TestClock) or a real regression in managed-policy propagation to running external plugins — needs a look on CI hardware to see whether it reproduces there.
