refactor(doctor): wire kernel reads directly

Summary

• delete ten one-implementation *DoctorAdapterV1 clone wrappers
• retain genuine Doctor source ports and canonical read mappers
• compose DoctorKernelInputsV1 directly from KernelDoctorSources in the root
• preserve coverage, availability and truth semantics

Tests

• contracts doctor adapters: 5 passed
• doctor report contracts: 11 passed
• advisory feedback contracts: 3 passed
• daemon doctor kernel: 12 passed
• affected check/clippy and deleted-name census clean
