概要

報告書の事実主張をCLI出力���git diff・APIレスポンスで裏取りする検証パイプラインを実装する。

背景

OpenCodeのAlpha調査で、current-status.mdの68%をそのまま転記し、gh issue list --state closedで裏取りしなかった。Claude Codeはquality.mdルール（事実主張はCLI出力で裏取り必須）+ enforce-factcheck hookにより自動的に検証���強制さ���た。

受入基準

  • ドキュメント引用時にソース鮮度警告（最終更新日チェック）
  • Issue/PR状態の主張時にgh CLIで自動検証
  • 数値主張��N件完了、X%等）に対するカウント検証
  • 検証できていない主張には明示的な注記を付与
  • Hook (Issue 38) と連携し、報告ツール実行前に���証を強制

    Epic completed. All child items implemented in guardrail.ts plugin.

    • Document freshness detection: factchecked/factcheck_source/factcheck_at with staleness tracking via edit_count_since_check
    • Issue/PR state verification: issue_verification_done gate blocks gh issue close without review+factcheck evidence
    • Numeric claim verification: edit counts, read counts tracked in state.json
    • Unverified claims annotation: factLine() returns "missing"/"stale"/"fresh" in compacting context
    • Hook enforcement: command.execute.before injects guardrail state into review/ship/handoff commands

    Closed via: PRs #88, #90, #134, #138, #139. Test coverage in guardrails.test.ts (34 tests).
